基于"痰瘀胜则阳气微"理论的慢性肺源性心脏病病机析微与辨治OA
Analysis of Pathogenesis and Treatment of Chronic Pulmonary Heart Disease Based on the Theory of"Dominance of Phlegm and Blood Stasis Leading to Yang Qi Deficiency"
慢性肺源性心脏病(Chronic Pulmonary Heart Disease,CPHD)是一种常见的多发性疾病,其发病机制复杂,严重影响患者生活质量.本文总结临床经验,提出"痰瘀胜则阳气微"为CPHD的核心病机,该病多以肺、脾、肾三脏亏虚为本,外邪侵袭、痰浊、瘀血为标.痰瘀与阳气之间为相互影响、互为因果的动态关系.痰瘀作为病理产物,阻滞气机并削弱阳气温煦推动作用,而阳气不足进一步加剧痰瘀积聚,形成恶性循环.治疗应遵循涤痰逐瘀、温阳化气、标本兼治的治疗原则,注意调节气血平衡,以期拓宽中医药治疗思路.
Chronic Pulmonary Heart Disease(CPHD)is a common and frequently occurring disease with complex pathogenesis,se-verely affecting patients' quality of life.This paper proposes that"the dominance of phlegm and Blood stasis leading to deficiency of Yang Qi"constitutes the core pathogenesis of CPHD.The disease is fundamentally characterized by deficiency of the Lung,Spleen,and Kidney,with external pathogen invasion,phlegm-turbidity,and Blood stasis as secondary manifestations.Phlegm,Blood stasis,and Yang Qi are in a dynamic relationship of mutual influence and causation:as pathological products,phlegm and Blood stasis block qi movement and weaken the warming and promoting effects of Yang Qi,while Yang deficiency further exacerbates the accumulation of phlegm and Blood stasis,forming a vicious cycle.Treatment should adhere to the principles of eliminating phlegm and removing Blood stasis,warming Yang and transforming Qi,and addressing both the root cause and superficial symptoms simultaneously.Attention should be paid to regulating the balance of Qi and Blood to broaden the therapeutic approaches of traditional Chinese medicine.
