秋水仙碱联合尼可地尔治疗冠状动脉慢血流型心绞痛的短期疗效及作用机制OA
Short-term efficacy and mechanism of colchicine combined with nicorandil in patients with coronary slow flow-type angina
目的:探讨秋水仙碱联合尼可地尔治疗冠状动脉慢血流(CSF)型心绞痛患者的短期疗效及作用机制.方法:选取接受治疗的120例CSF型心绞痛患者为研究对象,按照治疗方式不同将患者分为观察组(秋水仙碱联合尼可地尔)与对照组(予尼可地尔治疗),每组各60例.12周后,比较治疗效果、心绞痛发作情况、经校正心肌梗死溶栓试验帧数计数(CTFC)、内皮功能、炎症标志物、氧化应激指标及不良反应发生情况.结果:治疗后,观察组总有效率高于对照组(P<0.05).治疗后,两组患者的心绞痛发作频次、平均持续时间、左前降支(LAD)、左回旋支(LCX)、右冠状动脉(RCA)的CTFC、内皮素-1(ET-1)、高敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、髓过氧化物酶(MPO)、MPO/对氧磷酶(PON)均低于治疗前(P<0.05),且观察组均低于对照组(P<0.05).两组患者的一氧化氮(NO)、血流介导的血管舒张功能(FMD)均高于治疗前(P<0.05),且观察组均高于对照组(P<0.05).两组患者不良反应发生率无统计学差异(P>0.05).结论:秋水仙碱联合尼可地尔有利于提升CSF型心绞痛患者的短期疗效,其作用机制可能与改善血管内皮功能、抗炎及减轻氧化应激等协同作用相关.
Objective:To investigate the short-term efficacy and mechanism of colchicine combined with nicorandil in patients with coronary slow flow(CSF)-type angina.Methods:A total of 120 patients with CSF-type angina were selected and divided into an observation group(colchicine combined with nicorandil)and a control group(nicorandil alone)according to different treatment methods,with 60 cases in each group.After 12 weeks of treatment,the therapeutic effects,angina episodes,corrected Thrombolysis in Myocardial Infarction frame count(CTFC),endothelial function,inflammatory markers,oxidative stress indicators,and adverse reactions were compared between the two groups.Results:After 12 weeks of treatment,the total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the frequency and average duration of angina pectoris attacks,the corrected TIMI frame count(CTFC)of the left anterior descending artery(LAD),left circumflex artery(LCX),and right coronary artery(RCA),endothelin-1(ET-1),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α),myeloperoxidase(MPO)and the MPO/paraoxonase(PON)ratio in both groups were lower than those of before treatment(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).The levels of nitric oxide(NO)and flow-mediated dilation(FMD)in both groups were higher than those of before treatment(P<0.05),and those in the observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Colchicine combined with nicorandil improves the short-term efficacy in patients with CSF-type angina,and its mechanism may be related to synergistic effects such as improving vascular endothelial function,anti-inflammation,and reducing oxidative stress.
