微血流成像联合剪切波弹性成像在肺癌术前颈部淋巴结转移中的诊断价值OA
Diagnostic value of superb microvascular imaging combined with shear wave elastography in preoperative assessment of cervical lymph node metastasis in lung cancer
目的:探讨微血流成像(SMI)联合剪切波弹性成像(SWE)在肺癌术前颈部淋巴结转移中的诊断价值.方法:选取187例疑似肺癌伴颈部淋巴结肿大患者(共187枚淋巴结)为研究对象.所有患者术前均接受常规超声、超声SMI及SWE检查.以颈部淋巴结穿刺/活检病理或肺癌根治术后病理分期为标准,分别从患者层面(转移阳性组114例,转移阴性组73例)及淋巴结层面(转移组114枚,非转移组73枚)比较常规超声、SMI血流参数及SWE弹性参数;受试者工作特征曲线(ROC曲线)评估不同检查方法及联合应用对肺癌术前颈部淋巴结转移的诊断价值.结果:在患者层面,转移阳性组低分化比例高于转移阴性组(P<0.05).在淋巴结层面,转移组患者在常规超声上更多表现为纵横比<2、短径长、边界模糊、皮髓质分界消失、内部回声不均匀及微钙化(P<0.05);SMI显示转移组患者以周边型及混合型血流为主,非转移组患者以淋巴门型血流为主(P<0.05);转移组患者Adler血流分级Ⅱ~Ⅲ级比例高于非转移组(P<0.05);SWE定量参数显示,转移组患者杨氏模量最大值(Emax)、平均值(Emean)、最小值(Emin)及标准差(SD)高于非转移组(P<0.05).SMI与SWE联合检查诊断的敏感度、特异度、准确率分别为95.60%、90.40%、93.60%,AUC为0.938(95%CI:0.892~0.973),高于单一检查(P<0.05).结论:SMI可精细显示淋巴结内部血管构型,SWE能定量评估淋巴结硬度,二者从不同维度反映转移性淋巴结的病理特征.两者联合应用能提高肺癌术前颈部淋巴结转移的诊断准确性.
Objective:To investigate the diagnostic efficacy of the combined application of Superb Microvascular Imaging(SMI)and Shear Wave Elastography(SWE)in the preoperative evaluation of cervical lymph node metastasis in patients with lung cancer.Methods:A total of 187 patients(with 187 lymph nodes)with suspected lung cancer and cervical lymph node enlargement were selected as the study subjects.All patients underwent conventional ultrasound,superb microvascular imaging(SMI),and shear wave elastography(SWE)before surgery.Using cervical lymph node aspiration/biopsy pathology or pathological staging after radical lung cancer surgery as the gold standard,differences in conventional ultrasound,SMI blood flow parameters,and SWE elastography parameters were compared between the two groups at both the patient level(114 cases in the metastatic positive group and 73 cases in the metastatic negative group)and the lymph node level(114 lymph nodes in the metastatic group and 73 lymph nodes in the non-metastatic group).Receiver operating characteristic(ROC)curves were plotted to evaluate the diagnostic efficacy of different examination methods and their combined application.Results:At the patient level,the proportion of poor differentiation was higher in the metastatic positive group than in the metastatic negative group(P<0.05).At the lymph node level,metastatic nodes more frequently exhibited features on conventional ultrasound such as a long-axis/short-axis ratio<2,larger short-axis diameter,indistinct margins,loss of corticomedullary differentiation,heterogeneous internal echogenicity,and microcalcifications(P<0.05).SMI revealed that metastatic nodes predominantly showed peripheral or mixed vascular patterns,while non-metastatic nodes mainly showed hilar vascular patterns(P<0.05).The proportion of nodes with Adler blood flow grade II~III was higher in the metastatic group than that in the non-metastatic group(P<0.05).SWE quantitative parameters showed that the maximum Young's modulus(Emax),mean Young's modulus(Emean),minimum Young's modulus(Emin),and standard deviation(SD)were higher in the metastatic group compared to the non-metastatic group(P<0.05).The sensitivity,specificity,and accuracy of the combined SMI and SWE diagnosis were 95.60%,90.40%,and 93.60%,respectively,with an AUC of 0.938(95%CI:0.892~0.973),which were higher than those of any single method(P<0.05).Conclusion:SMI can finely depict the internal vascular architecture of lymph nodes,while SWE can quantitatively assess lymph node stiffness.Both techniques reflect the pathological characteristics of metastatic lymph nodes from different dimensions.Their combined application can improve the diagnostic accuracy for preoperative cervical lymph node metastasis in lung cancer.
