人群镉和铅暴露与慢性肾脏病发病风险的meta分析OA
Relationship between cadmium and lead exposure and chronic kidney disease:a meta-analysis
目的:通过meta分析探讨人群金属(镉、铅)暴露与慢性肾脏病发病风险之间的关系.方法:通过PubMed、Web of Science、The Cochrane Library、SinoMed、中国知网和万方数据库进行系统检索.Meta分析、敏感性分析和发表偏倚评价采用Stata 15.0软件进行分析.结果:共纳入24篇文献,21篇为镉暴露与慢性肾脏病相关,16篇为铅暴露与慢性肾脏病相关(其中13篇为以镉、铅暴露各自作为暴露变量的文献).Meta分析结果表明,人群镉暴露显著增加慢性肾脏病发病风险[OR=1.54,95%CI(1.33,1.79)],研究之间存在一定异质性(I2=76.5%,P<0.05),人群铅暴露与慢性肾脏病发病风险呈正相关[OR=1.46,95%CI(1.21,1.76)];研究之间存在一定异质性(I2=88.0%,P<0.05).敏感性分析显示本研究结果较稳定,纳入文献不存在明显发表偏倚.结论:人群镉、铅暴露与慢性肾脏病发病风险增加相关,金属镉和/或铅暴露是慢性肾脏病的显著风险因素.
OBJECTIVE:To investigate associations between metal exposure(cadmium and lead)and risk of chronic kidney disease(CKD)in populations through a meta-analysis.METHODS:A systematic search was conducted using PubMed,Web of Science,The Cochrane Library,SinoMed,CNKI and Wanfang databases.Meta-analysis,sensitivity analysis,and publication bias assessment were performed using Stata 15.0 software.RESULTS:A total of 24 studies were included,comprising 21 studies on associations between cadmium exposure and CKD and 16 studies on lead exposure.Among them,13 were reports that took cadmium and lead exposure as exposure variables.Our meta-analysis demonstrated that cadmium exposure in populations significantly increased risk of CKD[OR=1.54;95%CI(1.33,1.79)],with moderate heterogeneity observed among studies(I2=76.5%,P<0.05).Lead exposure was positively associated with CKD risk[OR=1.46;95%CI(1.21,1.76)],with significant heterogeneity among studies(I2=88.0%,P<0.05).Sensitivity analysis confirmed robustness of the findings.No significant publication bias was detected in the included studies.CONCLUSION:Cadmium and lead exposure in populations was associated with increased risk of CKD,indicating that metal exposure is a significant risk factor for CKD.
