靶向保护内皮糖萼为脓毒性心肌病提供新治疗策略OA
Targeted Protection of the Endothelial Glycocalyx Presents a Novel Therapeutic Strategy for Septic Cardiomyopathy
脓毒性心肌病是一种急性期死亡率极高的感染相关心脏疾病.内皮糖萼(EG)是覆盖于血管内皮细胞表面的重要结构,由蛋白聚糖、糖蛋白及其他生物大分子组成,在维持内皮细胞屏障功能和保护内皮结构完整性方面发挥关键作用.已有研究表明,脓毒症引发的免疫细胞异常活化与EG代谢失衡密切相关,可导致EG的脱落和降解,从而加重内皮功能障碍.因此,针对EG的保护与修复策略有望为脓毒性心肌病的药物治疗提供新方向.本文系统综述了EG的生理结构、其在脓毒性心肌病发病机制中的关键作用,以及EG损伤与炎症因子生成的相关机制.此外,本文还总结并探讨了以EG为靶点的潜在治疗策略,包括酶疗法、EG成分补充以及纳米材料相关治疗.
Septic cardiomyopathy is an infectious heart disease with a high mortality rate in the acute phase.The endothelial gly-cocalyx(EG)is an important structure that covers the surface of vascular endothelial cells.Composed of proteoglycans,glycoproteins,and other macromolecules,it plays a critical role in maintaining the barrier function of endothelial cells and protecting the structural integrity of the endothelium.Studies have shown that the activation of immune cells caused by sepsis is closely related to the metabolism of EG,which can lead to its structural shedding and degradation.Therefore,protective strategies against EG may provide a new direc-tion for drug therapy in septic cardiomyopathy.The purpose of this paper is to review the physiological structure of EG,its key role in the pathogenesis of septic cardiomyopathy,and its related mechanism to the production of inflammatory factors.In addition,this review includes treatment options around EG production,such as enzymatic therapy,supplementation of endothelial glycocalyx components,and treatment of nanomaterials.
