免疫-细胞外基质(ECM)轴失衡是主动脉夹层的核心病理机制,免疫异常活化与ECM降解重构共同破坏主动脉壁结构稳定性.多组学研究揭示了其分子特征:基因组学鉴定出ACTA2、FBN1等易感基因;转录组、蛋白质组及代谢组分别发现表观调控、关键效应分子(如MMPs、S100A8/A9)及代谢紊乱参与疾病进程.血管周围脂肪组织(PVAT)驱动炎症放大,单核/巨噬细胞异常浸润及细胞因子风暴促进免疫失调,MMPs过度激活则介导胶原与弹性蛋白降解.炎症-ECM交互、机械应力-免疫反馈构成关键致病环路,单细胞技术进一步解析细胞互作网络.目前已建立多组学驱动的早期诊断与靶向治疗方向,但仍面临人群异质性、模型转化差异等挑战.未来需借助人工智能与时空多组学,推进主动脉夹层精准分型与个体化诊疗.

Dysregulation of the immune-extracellular matrix(ECM)axis is the central pathological mechanism of aortic dissec-tion,where abnormal immune activation and ECM degradation and remodeling collectively compromise the structural integrity of the aortic wall.Multi-omics studies have revealed its molecular characteristics:genomics has identified susceptibility genes such as ACTA2 and FBN1;transcriptomics,proteomics,and metabolomics have respectively identified epigenetic regulation,key effector mol-ecules(such as MMPs and S100A8/A9),and metabolic dysregulation involved in the disease process.Perivascular adipose tissue(PVAT)drives inflammatory amplification,abnormal monocyte/macrophage infiltration and cytokine storms promote immune dysregu-lation,and excessive MMP activation mediates collagen and elastin degradation.The inflammation-ECM interaction and mechanical stress-immune feedback constitute key pathogenic loops,and single-cell technologies further dissect cellular interaction networks.Although multi-omics-driven strategies for early diagnosis and targeted therapy have been established,challenges such as population heterogeneity and model translation discrepancies remain.Future efforts should leverage artificial intelligence and spatiotemporal multi-omics to advance precise subclassification and personalized diagnosis and treatment of aortic dissection.
