Advancements in research on types of excipients of inhaled preparations

药用辅料是决定肺吸入制剂性能与安全的关键.该文对吸入气雾剂、吸入喷雾剂和吸入粉雾剂的关键辅料研究进展进行了综述.吸入气雾剂的核心辅料包括作为动力的抛射剂(正向新型环保抛射剂发展)、提高溶解度的助溶剂/增溶剂(如乙醇),以及维持理化性质稳定的稳定剂(如表面活性剂、抗氧剂).吸入喷雾剂为溶液或混悬体系,重点在于选用合适的助溶剂和稳定剂以保证药物的溶解性、无菌性和稳定性.吸入粉雾剂则不依赖抛射剂,其效能核心在于载体辅料(如乳糖)的粒径与形态、分散增强剂(如L-亮氨酸)的应用,以及保证产品稳定的稳定剂系统.未来,辅料研发将趋向功能化与智能化,尤其新型纳米辅料与核酸递送系统的发展,将为肺部大分子药物递送提供新的解决方案.

Pharmaceutical excipients are critical to the performance and safety of pulmonary inhaled formulations.This article reviews the research advances in key excipients of three dominating types of inhaled preparations:pressurized metered-dose inhalers,nebulizers,and dry powder inhalers.The first type contains propellants(becoming novel and environmentally friendly)that serve as the driving force,cosolvents/solubilizers(e.g.,ethanol)that enhance drug solubility,and stabilizers(e.g.,surfactants,antioxidants)that maintain physical and chemical stability.The second type-nebulizers-are solutions or suspension systems that rely on appropriate cosolvents and stabilizers to ensure the solubility,sterility and stability of drugs.Finally,dry powder inhalers,which require no propellants,depend on the properties of carrier excipients(e.g.,lactose),the use of dispersion enhancers(e.g.,L-leucine),and stabilizer systems to ensure stability of products.The research and development of excipients will become increasingly functional and intelligent.Advances in novel nano-excipients and nucleic acid delivery systems are expected to provide new solutions to the pulmonary delivery of macromolecular drugs.
