目的:研究听觉注意力—执行训练对脑外伤后认知障碍及心理状态的临床疗效.方法:选取2024年1月—2024年12月在某医院住院的76例脑外伤后认知功能障碍患者,采用随机数字表法分为对照组和试验组,每组38例.对照组采用常规训练,包括经颅直流电刺激、注意力训练、执行功能训练、记忆训练.试验组给予经颅直流电刺激、听觉注意力—执行训练、记忆训练.两组分别在治疗前、治疗6周后采用简易精神状态检查量表(Mini—mental State Examination,MMSE)、蒙特利乐认知功能评估(Montreal Cognitive Assessment,MOCA)、改良 Bar-thel 指数(Modified Barthel Index,MBI)、数字广度测试(Digit Span Test,DST)、连线测验(Trail-Making Test,TMT)、修订韦氏成人记忆量表(WMS-RC)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)进行双盲评定,比较两组的认知功能和心理状态.结果:两组在治疗前的MMSE、MoCA、MBI、DST、TMT、WMS-RC、HAMA、HAMD评分组间差异无统计学意义(P>0.05),两组治疗后的 MMSE、MoCA、MBI、DST、TMT、WMS-RC、HAMA、HAMD 评分均优于治疗前(Z=5.272,4.576,4.424,5.246,4.613,-5.379,-4.940,-4.619,5.184,t=-12.120,-6.372,-5.957,Z=5.387,5.169,5.430,5.472,5.397,-5.375,-5.136,-5.275,5.376,t=-12.549,-9.578,-7.506;P<0.001),差异有统计学意义,且观察组的 MMSE、MoCA、MBI、DST、TMT、WMS-RC、HAMA、HAMD 分值变化均优于对照组(Z=4.264,-4.91,3.420,4.684,3.347,-2.340,-2.803,-5.008,3.255,t=2.325,2.323,2.267;P<0.05),差异有统计学意义.结论:听觉注意力—执行训练能够有效改善脑外伤患者的认知障碍及心理状态.

Objective:To investigate the clinical effect of auditory attention-execution training on cognitive impair-ment and psychological state after traumatic brain injury.Methods:A total of 76 patients with cognitive dysfunction after traumatic brain injury in our hospital from January 2024 to December 2024 were selected and divided into a control group and an observation group by random number table method,with 38 cases in each group.The control group received routine training:Transcranial direct current stimulation,attention training,executive function training and memory training.The observation group was given transcranial direct current stimulation,auditory attention-execution training and memory training.All patients were assessed by double-blind using the Mini-Mental State Examination(MMSE),Montreal Cognitive Assessment(MOCA),Modified Barthel Index(MBI),Digit Span Test(DST),Trail-Making Test(TMT),Revised Wechsler Memory Scale(WMS-RC),Hamilton Anxiety Scale(HA-MA),and Hamilton Depression Scale(HAMD)before treatment and after 6 weeks treatment,in order to compare the cognitive and psychological status of the 2 groups.Results:Before intervention,there were no significant differences between the 2 groups in any of the assessment(P>0.05).After treatment,the MMSE,MoCA,MBI,DST,TMT,WMS-RC,HAMA,HAMD scores of all 2 groups were better than before treatment(Z=5.272,4.576,4.424,5.246,4.613,-5.379,-4.940,-4.619,5.184,t=-12.120,-6.372,-5.957;Z=5.387,5.169,5.430,5.472,5.397,-5.375,-5.136,-5.275,5.376,t=-12.549,-9.578,-7.506;P<0.001).Moreover,the MMSE,MoCA,MBI,DST,TMT,WMS-RC,HAMA,HAMD score changes of observation group were better than those of control group,and the differ-ence was statistically significant(Z=4.264,-4.91,3.420,4.684,3.347,-2.340,-2.803,-5.008,3.255;t=2.325,2.323,2.267;P<0.05).Conclusion:Auditory attention-executive training can effectively improve the cognitive impairment and psychological state of patients with traumatic brain injury.
