2020-2024年重庆市渝北区肺结核流行特征及与大气污染物的相关性分析OA
Epidemiological characteristics of pulmonary tuberculosis and its correlation with air pollutants in Yubei District,Chongqing,2020-2024
目的 描述重庆市渝北区肺结核流行特征,分析大气污染物对肺结核发病风险的影响,为该病的防控、医疗保健和卫生决策提供理论依据.方法 数据来自中国疾病预防控制信息系统传染病监测报告系统、渝北区统计年鉴以及中国环境监测总站.采用描述性流行病学研究方法、Joinpoint回归模型、空间自相关以及Spearman等级相关,对重庆市渝北区肺结核流行特征及其与大气污染物的相关性进行分析.结果 2020-2024年重庆市渝北区累计报告肺结核病例5 423例,年均报告发病率为56.50/10万,总体呈下降趋势[平均年度变化百分比(AAPC)=-19.99%,P<0.05].三间分布显示,2020-2024年重庆市渝北区男性和女性肺结核报告发病率均呈下降趋势(男性 AAPC=-19.33%,P<0.05;女性 AAPC=-21.04%,P<0.05),且男性年均报告发病率高于女性(χ2=556.653,P<0.001);全年龄组中,肺结核年均报告发病率前3位依次为80~89岁组(278.88/10万)、70~79岁组(237.57/10万)和90岁及以上组(205.34/10万);2020-2024年,重庆市渝北区肺结核病例主要集中在家务及待业人员、农民及其他职业人群中,以上群体中的肺结核病例占总病例数的70.18%;2020-2024年,重庆市渝北区肺结核发病呈散发状态,未见明显季节性聚集;2020-2024年,重庆市渝北区肺结核年平均报告发病率较高的镇街主要分布于距中心城区较远的地区,居前3位者依次为兴隆镇(105.46/10万)、古路镇(104.85/10万)和木耳镇(99.49/10万).空间自相关分析结果显示,2021-2024年渝北区肺结核报告发病率均存在空间聚集性,呈空间正相关,但空间依赖性在逐渐减弱,高-高聚集区主要集中在距中心城区较远的镇街如兴隆镇、大湾镇、统景镇等.Spearman相关分析显示,肺结核日发病数与 NO2、CO水平呈正相关(P<0.05).结论 2020-2024年重庆市渝北区肺结核报告发病率总体呈下降趋势.应重点加强男性、70岁及以上老年人、农村和偏远地区人群等重点人群的防控工作.此外,肺结核的发生和流行与大气污染物之间存在相关性.
Objective To describe the epidemiological characteristics of pulmonary tuberculosis in Yubei District,Chongqing,and to analyze the impact of air pollutants on the risk of its incidence,so as to provide a theoretical basis for disease preven-tion and control,healthcare,and health decision-making.Methods Data were obtained from the Infectious Disease Surveil-lance and Reporting System of the China Disease Prevention and Control Information System,the Yubei District Statistical Yearbook,and the China National Environmental Monitoring Centre.Descriptive epidemiological methods,Joinpoint re-gression models,spatial autocorrelation analysis,and Spearman rank correlation analysis were used to analyze the epidemio-logical characteristics of pulmonary tuberculosis in Yubei District,and its correlation with air pollutants.Results From 2020 to 2024,a total of 5,423 pulmonary tuberculosis cases were reported in Yubei District,with an average annual reported incidence of 56.50/100,000,showing an overall downward trend(average annual percent change[AAPC]=-19.99%,P<0.05).The person-place-time distribution showed that the reported incidence of pulmonary tuberculosis in both males and females in Yubei District,from 2020 to 2024 showed a downward trend(male:AAPC=-19.33%,P<0.05;female:AAPC=-21.04%,P<0.05),and the average annual reported incidence in males was higher than that in females(χ2=556.653,P<0.001).Among all age groups,the top three average annual reported incidences of pulmonary tuberculosis were in the 80-89 years group(278.88/100,000),the 70-79 years group(237.57/100,000),and the≥90 years group(205.34/100,000).From 2020 to 2024,pulmonary tuberculosis cases in Yubei District were mainly concentrated among houseworkers and unemployed individuals,farmers,and other occupational groups,accounting for 70.18%of the total ca-ses.From 2020 to 2024,pulmonary tuberculosis incidence in Yubei District was sporadic,with no marked seasonal cluste-ring.From 2020 to 2024,the towns and subdistricts with higher average annual reported incidences of pulmonary tuberculosis in Yubei District were mainly distributed in areas farther from the central urban area,with the top three being Xinglong Township(105.46/100,000),Gulu Township(104.85/100,000),and Muer Township(99.49/100,000).Spatial autocorrelation analysis showed that the reported incidence of pulmonary tuberculosis in Yubei District from 2021 to 2024 all exhibited spatial cluste-ring,showing positive spatial correlation,but the spatial dependence gradually weakened.The high-high clustering areas were mainly concentrated in towns and subdistricts farther from the central urban area,such as Xinglong Township,Dawan Township,and Tongjing Township.Spearman correlation analysis showed that the daily number of pulmonary tuberculosis cases was positively correlated with NO2 and CO levels(P<0.05).Conclusion The reported incidence of pulmonary tubercu-losis in Yubei District,Chongqing,from 2020 to 2024 showed an overall downward trend.Prevention and control efforts should be focused on key populations,including males,the elderly aged 70 years and above,and people in rural and remote areas.In addition,there is a correlation between the occurrence and spread of pulmonary tuberculosis and air pollutants.
