年轻乳腺癌患者生育力保存及辅助生殖的现状与展望OA

Fertility preservation and assisted reproductive technologies in young patients with breast cancer:current status and future perspectives

育龄期乳腺癌患者的生育力保存已成为肿瘤生殖学领域的重要议题.随着诊疗水平提升,年轻患者的生育需求日益凸显,生育力保存策略也从单一的胚胎冷冻,发展为包含卵母细胞冷冻、卵巢组织冷冻在内的多元化技术体系.关键性临床研究,特别是POSITIVE研究,为激素受体阳性患者中断内分泌治疗以实现生育的短期安全性提供了高级别证据,改变了临床实践.目前,临床争论与热点集中于最佳妊娠时机、个体化技术路径选择以及遗传性乳腺癌(如BRCA基因突变携带者)的生殖风险管理.多学科协作模式是成功实施生育力保存的基石.但是,该领域仍面临临床实践规范化不足、部分先进技术可及性有限以及长期随访数据有待完善等挑战.未来,整合精准医学理念、强化全程管理并完善政策支持体系,是实现"肿瘤治疗"与"生育愿望"平衡共赢的关键方向.

Fertility preservation in women of reproductive age with breast cancer has become a critical issue in the field of oncofertility.With advances in cancer diagnosis and treatment,the demand for fertility preservation among young patients has increased substantially.Correspondingly,available strategies have evolved from embryo cryopreservation alone to a diversified technological framework that includes oocyte cryopreservation and ovarian tissue cryopreservation.Key clin-ical studies,particularly the POSITIVE trial,have provided high-level evidence supporting the short-term safety of in-terrupting endocrine therapy to achieve pregnancy in patients with hormone receptor-positive breast cancer,thereby influ-encing clinical practice.Current controversies and research hotspots focus on the optimal timing of pregnancy,individual-ized selection of fertility preservation strategies,and reproductive risk management in patients with hereditary breast canc-er,such as BRCA mutation carriers.A multidisciplinary approach is essential for the successful implementation of fertility preservation.However,several challenges remain,including insufficient standardization of clinical practice,limited ac-cessibility to advanced technologies,and a lack of long-term follow-up data.Future efforts should emphasize the inte-gration of precision medicine,optimization of longitudinal patient management,and improvement of supportive policy frameworks,in order to achieve a balanced outcome between effective cancer treatment and fertility goals.
