本文基于李晶教授提出的食管癌核心病机"胃脘干槁,血液俱耗",确立"甘润濡养"为治疗法则,系统探讨食管癌的演变规律与辨证思路.结合文献梳理与临床实践,提出分期辨证论治:津伤期治以甘寒生津,润燥启膈;液损期治以甘润养液,益胃养阴;血亏期治以养阴补血,滋补肝肾;精耗期治以补髓填精,濡养五脏.实践表明,该病机指导下的辨证方案可改善症状、延长生存期,体现中医"辨证论治"与"整体观念"优势.

Based on the pathogenesis of"dryness and exhaustion of the gastric cavity with depletion of both blood and fluid"proposed by Professor Li Jing,this paper systematically explores the evolution law and syndrome differentiation strategy of esophageal cancer.By integrating literature review and clinical practice,this paper puts forward staged syndrome differentiation and treatment:for the fluid injury stage,promoting fluid production with sweet-cold medicinals,moistening dryness and unblocking the diaphragm;for the fluid depletion stage,nourishing fluid with sweet-moist medicinals,invigorating stomach and nourishing yin;for the blood deficiency stage,nourishing yin and replenishing blood,and tonifying liver and kidney;for the essence exhaustion stage,tonifying marrow and replenishing essence,and nourishing five zang-organs.Clinical practice shows that the syndrome differentiation protocol guided by this pathogenesis can alleviate clinical symptoms and prolong survival time,reflecting the advantages of traditional Chinese medicine in syndrome differentiation and treatment and the holistic concept.
