肝细胞肝癌(hepatocellular carcinoma,HCC)是全球高发病率、高死亡率的恶性肿瘤,其预后不良主要与肝外转移密切相关.全国名中医凌昌全教授提出"癌毒盛衰"是肝癌发生发展的核心病机."癌毒"损伤机体正气,而正虚无力约束癌毒,以致癌毒扩散,继而损伤靶器官正气.现代研究发现,肝癌细胞分泌的外泌体、细胞因子等,诱导转移靶器官"转移前生态位"(pre-metastatic niche,PMN)形成.免疫抑制是PMN的典型特征之一,也是中医学"正虚"理论的生物学基础之一.局部正虚不能抑制流窜之癌毒,以致"虚毒互结",加速肿瘤转移的发生.本文基于中西医整合视角,构建"癌毒—正虚—PMN形成—免疫抑制—肝癌转移"的理论框架,揭示中医"因毒致虚"病机在靶器官局部的生物学基础,为肝癌转移的防治提供新思路与潜在靶点.

Hepatocellular carcinoma(HCC)is a malignant tumor with high incidence rate and mortality globally,and its poor prognosis is closely related to extrahepatic metastasis.Professor Ling Changquan,a nationally renowned traditional Chinese medicine practitioner,proposed that"the rise and fall of cancer toxin"is the core pathogenesis of the occurrence and development of liver cancer."Cancer toxin"damages the body's vital qi,and the deficiency of vital qi cannot restrain cancer toxin,resulting in the spread of cancer toxin and the subsequent impairment of vital qi of target organs.Modern research has found that exosomes,cytokines,and other substances secreted by hepatocellular carcinoma cells induce the formation of a"pre-metastatic niche(PMN)"in the target organs of metastasis,and immune suppression is one of the typical characteristics of PMN,and also one of the biological foundations of the"deficiency of vital qi"theory in traditional Chinese medicine.Local vital qi deficiency fails to suppress the spreading cancer toxin,leading to a"deficiency-toxin mutual aggravation"that accelerates tumor metastasis.Based on the perspective of integrating traditional Chinese and Western medicine,this article constructs a theoretical framework of"cancer toxin-deficiency of vital qi-PMN formation-immune suppression-liver cancer metastasis",reveals the biological basis of the pathogenesis of"deficiency caused by toxin"in traditional Chinese medicine at the local level of the target organs,and provides new ideas and potential targets for the prevention and treatment of liver cancer metastasis.
