目的:基于网络药理学预测加味小承气汤治疗肝癌轻微肝性脑病的机制,采用临床实验验证加味小承气汤对肝癌轻微肝性脑病症状的改善作用.方法:通过网络药理学分析加味小承气汤灌肠治疗轻微肝性脑病的核心靶点及通路.选取2024 年6 月-2025 年10 月在四川省中西医结合医院肿瘤科病理诊断为肝细胞癌,且数字连接试验-A、数字符号试验均为阳性,诊断为肝癌轻微型肝性脑病的患者.研究采用前瞻性随机对照设计,应用区组随机化方法将受试者分为对照组与观察组,在研究期间,两组均使用常规治疗方案,其中观察组加用加味小承气汤灌肠,对照组以乳果糖灌肠,治疗2 周后,比较两组患者的有效率;检测两组患者的血清炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)与 IL-1β]含量,以及肝功能指标[血氨、天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)和总胆红素(TBil)]含量或活力;采用数字符号试验与数字连接试验-A 对患者的认知功能状况进行评估;通过中医证候积分评价患者临床症状改善程度.结果:网络药理学分析显示,加味小承气汤中的108 个活性成分治疗靶点有1 007 个,肝性脑病相关靶点有950 个;交集靶点有289 个;经GO 和KEGG 富集分析,得到2 677 个功能条目与 234 条显著信号通路,MAPK、NF-κB 等关键信号通路是发挥治疗作用的分子基础.临床试验表明:治疗后,与对照组比较,观察组患者血清 TNF-α、IL-6、IL-1β 及血氨含量明显降低(P<0.05),数字符号试验认识数目升高(P<0.05),数字连接试验-A 用时缩短(P<0.05),中医证候积分明显降低(P<0.05).结论:加味小承气汤治疗肝癌轻微型肝性脑病疗效确切,可有效调节炎症因子水平,降低血氨水平,缩短认知功能恢复时间,改善肝功能,其机制可能是通过调控 MAPK、NF-κB 等信号通路,降低炎症因子水平并促进血氨代谢有关.

Objective:To predict the mechanism of modified Xiaochengqi Decoction(小承气汤)in the treatment of mild hepatic encephalopathy in liver cancer based on network pharmacology,and to verify its improving effect on symp-toms of mild hepatic encephalopathy in liver cancer by clinical experiments.Methods:The core targets and pathways of treating mild hepatic encephalopathy by retention enema with modified Xiaochengqi Decoction were studied by network pharmacology analysis.Patients with hepatocellular carcinoma diagnosed by pathology in the Department of Oncology at Sichuan Integrative Medicine Hospital from June 2024 to October 2025 were selected,who tested positive on both the number connection test-A(NCT-A)and digit symbol test(DST)and were diagnosed with mild hepatic encephalopathy in liver cancer.They were equally divided into a control group and an experimental group by using a prospective random-ized controlled design and employing the block randomization method.The patients in both groups were given convention-al treatment during the study period.Modified Xiaochengqi Decoction was used for retention enema in the experimental group,while lactulose was adopted for retention enema in the control group.After 2 weeks of treatment,the efficacy rates of both groups were compared.The serum levels of inflammatory factors(tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-1β(IL-1β))and the content and activities of liver function indicators(blood ammonia,as-partate transaminase(AST),alanine transaminase(ALT),and total bilirubin(TBIL))were detected.The cognitive function of the two groups was evaluated by DST and NCT-A,and the improvement of clinical symptoms was evaluated by traditional Chinese medicine(TCM)syndrome score.Results:The network pharmacology analysis showed that there were 1007 therapeutic targets corresponding to the 108 active ingredients in modified Xiaochengqi Decoction,950 targets related to hepatic encephalopathy,and 289 overlapping targets.By GO and KEGG enrichment analysis,2667 function en-tries and 234 significant signal pathways were obtained(P<0.05).Key signaling pathways such as MAPK and NF-κB were the molecular basis for the therapeutic effects.After treatment,compared with the control group,the experimental group showed reduced serum levels of TNF-α,IL-6,IL-1β,and blood ammonia(P<0.05),an increase in the number of correct responses on the DST(P<0.05),a decrease in the completion time of NCT-A(P<0.05),and an obviously re-duced TCM syndrome score(P<0.05).Conclusion:The modified Xiaochengqi Decoction is effective in the treatment of mild hepatic encephalopathy in liver cancer,which can effectively regulate the levels of inflammatory factors,reduce the level of blood ammonia,shorten the recovery time of cognitive function,and improve liver function.Its mechanism may involve reducing inflammatory factor levels and promoting blood ammonia metabolism through MAPK,NF-κB,and other signaling pathways.
