基于TLR4/MYD88/NF-κB信号通路和肠道菌群探讨罗乐胃蜜丸对慢性非萎缩性胃炎大鼠的作用及机制OA
Effects and Mechanism of Luolewei Honey Pills on Chronic Non-Atrophic Gastritis in Rats Based on TLR4/MYD88/NF-κB Signaling Pathway and Gut Microbiota
目的:探讨罗乐胃蜜丸对慢性非萎缩性胃炎(chronic non atrophic gastritis,CNAG)大鼠的治疗作用及潜在机制.方法:采用多因素联合刺激法建立 CNAG 大鼠模型,将造模成功大鼠随机分为模型对照组、罗乐胃蜜丸0.2、0.4、0.8 g/kg 组、胃苏颗粒4.7 g/kg 组,另设正常对照组,各给药组灌胃给予相应药物 4 w.观察大鼠一般状态及体质量变化;测定胃液pH 值及胃蛋白酶活性;利用酶联免疫吸附(ELISA)法测定血清中白介素-6(IL-6)、IL-1β、肿瘤坏死因子-α(TNF-α)、胃泌素(GAS)、胃动素(MTL)含量;HE 染色观察胃黏膜组织病理学变化;Western blot 法检测胃组织 TLR4、MYD88、NF-κB p65 蛋白表达;16S rRNA 高通量测序分析粪便的肠道菌群结构变化,并关联菌群与炎症因子、胃肠激素的相关性.采用正常小鼠和阿托品致胃肠动力障碍模型小鼠,观察罗乐胃蜜丸对胃排空率和小肠推进率的影响.结果:与正常对照组比较,模型对照组大鼠体质量增长缓慢,胃蛋白酶活力显著降低(P<0.01),胃液 pH 值、血清 IL-6、IL-1β、TNF-α 及 GAS 含量显著升高(P<0.01),MTL 含量显著降低(P<0.01),胃黏膜出现明显病理损伤,胃组织 TLR4、MYD88、NF-κB p65 蛋白表达显著上调(P<0.01),肠道菌群多样性改变,拟杆菌门等有益菌相对丰度降低,假单胞菌门等有害菌相对丰度升高;与模型对照组比较,罗乐胃蜜丸组大鼠体质量、胃蛋白酶活力显著升高(P<0.01),胃液 pH 值及血清 IL-6、IL-1β、TNF-α、GAS 含量明显降低(P<0.05 或 P<0.01),MTL 含量显著升高(P<0.01),胃黏膜组织病理损伤改善,TLR4、MYD88、NF-κB p65 蛋白表达明显下调(P<0.05 或P<0.01);罗乐胃蜜丸各组杆菌门等有益菌相对丰度升高,假单胞菌门等有害菌相对丰度降低;保护性菌群(如拟杆菌门)丰度与炎症因子、GAS 呈负相关,与 MTL 呈正相关,而损害型菌群(如假单胞菌门)丰度与之相反.罗乐胃蜜丸各组均能显著提高阿托品致胃肠动力障碍模型小鼠的胃排空率和小肠推进率(P<0.05 或 P<0.01).结论:罗乐胃蜜丸可有效改善 CNAG 大鼠胃黏膜炎性损伤,其机制可能与抑制 TLR4/MYD88/NF-κB 信号通路激活、减轻炎症反应、调节胃肠激素及重塑肠道菌群结构有关.
Objective:This paper aims to investigate the therapeutic effects and potential mechanism of Luolewei Honey Pills(罗乐胃蜜丸)on chronic non-atrophic gastritis(CNAG)rats.Methods:The CNAG rat model was estab-lished by a multi-factor combined stimulation method.The successfully modeled rats were randomly divided into a model control group,Luolewei Honey Pill groups(0.2,0.4,and 0.8 g/kg),and a Weisu Granule group(4.7 g/kg),and a normal control group was set.Each group received intragastric administration for four weeks.The general state and body mass changes of the rats were observed.The gastric juice pH value and pepsin activity were measured.The levels of in-terleukin-6(IL-6),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),gastrin(GAS),and motilin(MTL)in serum were determined by enzyme-linked immunosorbent assay(ELISA).Histopathological changes of the gastric muco-sa were observed by hematoxylin-eosin(HE)staining.The protein expressions of toll-like receptor 4(TLR4),myeloid differentiation primary response protein 88(MYD88),and nuclear factor kappa-B p65(NF-κB p65)in gastric tissue were detected by Western blot.Changes in fecal intestinal microbiota structure were analyzed by 16S rRNA high-through-put sequencing,and the correlations between microbiota and inflammatory factors,as well as gastrointestinal hormones,were evaluated.In addition,normal mice and model mice with gastrointestinal motility disorder induced by atropine were used to observe the effects of Luolewei Honey Pills on gastric emptying rate and small intestinal propulsion rate.Results:Compared with those in the normal control group,the rats in the model control group showed significantly slower body weight gain and significantly decreased pepsin activity(P<0.01).The gastric juice pH value and the levels of IL-6,IL-1β,TNF-α,and GAS in serum were increased(P<0.01),and the MTL content was significantly decreased(P<0.01).Obvious pathological damage was observed in the gastric mucosa.Protein expressions of TLR4,MYD88,and NF-κB p65 were significantly up-regulated(P<0.01).Intestinal microbial diversity was altered,characterized by a decreased rela-tive abundance of beneficial bacteria such as Bacteroidetes and an increased relative abundance of harmful bacteria such as Proteobacteria.Compared with those in the model control group,the body weight and pepsin activity of rats in the Lu-olewei Honey Pill groups were significantly increased(P<0.01),while gastric juice pH value and the contents of IL-6,IL-1β,TNF-α,and GAS in serum were decreased(P<0.05 or P<0.01).The MTL content was significantly increased(P<0.01).In addition,the pathological damage of the gastric mucosa was alleviated,and the protein expressions of TLR4,MYD88,and NF-κB p65 were down-regulated(P<0.05 or P<0.01).Luolewei Honey Pills increased the relative abundance of beneficial bacteria like Bacteroidetes and decreased the relative abundance of harmful bacteria like Pro-teobacteria.The abundance of protective microbiota(e.g.,Bacteroidetes)was negatively correlated with inflammatory factors and GAS and positively correlated with MTL,while the abundance of harmful microbiota(e.g.,Proteobacteria)showed the opposite correlations.Luolewei Honey Pills significantly increased the gastric emptying rate and small intesti-nal propulsion rate in the model mice with gastrointestinal motility disorder induced by atropine(P<0.05 or P<0.01).Conclusion:Luolewei Honey Pills can effectively ameliorate gastric mucosal inflammatory injury in CNAG rats.The mechanism may be related to inhibiting the activation of the TLR4/MYD88/NF-κB signaling pathway,alleviating inflam-matory responses,regulating gastrointestinal hormones,and remodeling the intestinal microbiota structure.
