TNF-α相关炎症机制在绝经后骨质疏松症中的作用及中药活性成分、单味药、复方干预的研究进展OA
The Role of TNF-α-Related Inflammatory Mechanisms in Postmenopausal Osteoporosis and the Research Progress of Interventions with Active Components,Single Herbs,and Compound Formulas of Chinese Medicine
绝经后骨质疏松症(Postmenopausal Osteoporosis,PMOP)是中老年女性中最常见的骨代谢性疾病,其发生发展与雌激素缺乏诱导的炎症免疫异常密切相关.随着骨免疫学理论的发展,肿瘤坏死因子-α(TNF-α)被认为是连接免疫激活与骨重塑失衡的重要促炎细胞因子.雌激素缺乏可通过多层级免疫调控机制促进 TNF-α异常激活,并经 NF-κB、MAPK、PI3K/AKT 等信号通路增强破骨细胞分化和骨吸收、抑制成骨功能,最终导致骨重塑失衡.同时,TNF-α 抑制剂可改善炎症性疾病人群的骨代谢指标并提高骨密度,但其在长期安全性、经济负担及骨折结局获益等方面仍存在局限.已有研究显示,在中药干预 PMOP 过程中,TNF-α 等炎症因子水平下降常与骨代谢改善现象同步发生,提示 TNF-α 可能是中药作用过程中的重要关联因子之一.本文主要综述 TNF-α 在PMOP 中的发病机制,整理中药干预PMOP 过程中的TNF-α 变化特征及其相关信号通路,为进一步阐明中药治疗机制提供理论参考.
Postmenopausal osteoporosis(PMOP)is the most common metabolic bone disease in middle-aged and elderly women,and its occurrence and progression are closely associated with inflammatory immune abnormalities induced by estrogen deficiency.With the development of osteoimmunology,tumor necrosis factor-α(TNF-α)has been recognized as an important proinflammatory cytokine linking immune activation to bone remodeling imbalance.Estrogen deficiency promotes aberrant activation of TNF-α through multilayered immune regulatory mechanisms and further enhances osteo-clast differentiation and bone resorption while suppressing osteoblast function through signaling pathways such as nuclear factor kappa B(NF-κB),Mitogen-activated protein kinase(MAPK),and phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt),ultimately leading to bone remodeling imbalance.Meanwhile,TNF-α inhibitors have been shown to im-prove bone metabolic markers and increase bone mineral density in patients with inflammatory diseases;however,their long-term safety,economic burden,and benefits on fracture outcomes remain limited.Existing studies have shown that re-ductions in inflammatory factors such as TNF-α during traditional Chinese medicine(TCM)interventions for PMOP are often accompanied by improvements in bone metabolism,suggesting that TNF-α may be one of the important associated factors in the therapeutic effects of TCM.This review summarizes the role of TNF-α in the pathogenesis of PMOP and outlines the characteristics of TNF-α changes and the related signaling pathways during TCM interventions,with the aim of providing a theoretical reference for further elucidating the therapeutic mechanisms of TCM.
