化疗联合经典名方小柴胡汤治疗乳腺癌疗效及安全性的Meta分析OA
Meta-Analysis of Efficacy and Safety of Chemotherapy Combined with the Classic Prescription Xiaochaihu Decoction for Treating Breast Cancer
目的:运用 Meta 分析系统评价化疗联合经典名方小柴胡汤治疗乳腺癌的有效性及安全性,并提供循证医学证据支持.方法:在 PubMed、Embase、The Cochrane Library、Web of science、中国知网、维普、万方和中国生物医学文献数据库中检索 2026 年 1 月 1 日以前有关化疗联合小柴胡汤治疗乳腺癌的随机对照研究,以RevMan5.3 软件分析数据.结果:纳入17 项研究,患者 1 292 例.Meta 分析结果显示:对比单纯化疗,化疗联合小柴胡汤能够提高临床有效率[RR=1.45,95%CI(1.30,1.62),P<0.01],降低糖类抗原 125(CA125)[MD=-41.26,95%CI(-53.32,-29.20),P<0.01]、降低糖类抗原 153(CA153)[MD=-15.28,95%CI(-16.81,-13.75),P<0.01]、降低癌胚抗原(CEA)[MD=-3.73,95%CI(-4.23,-3.24),P<0.01]、降低胰岛素样生长因子-1(IGF-1)[MD=-58.09,95%CI(-59.97,-56.21),P<0.01]、降低可溶性 E-钙黏连蛋白(sEC)[MD=-0.79,95%CI(-0.82,-0.77),P<0.01],提高 T 淋巴细胞(CD3+、CD4+、CD4+/CD8+)[MD=0.31,95%CI(0.25,0.36),P<0.01],抑制 CD8+[MD=-5.88,95%CI(-6.52,-5.24),P<0.01],提高免疫球蛋白 A、M、G(IgA、IgM、IgG)[MD=0.50,95%CI(0.47,0.53),P<0.01],降低肿瘤血管内皮生长因子 A[MD=-6.98,95%CI(-8.68,-5.28),P<0.01]及肿瘤血管内皮生长因子 B[MD=-5.82,95%CI(-7.73,-3.90),P<0.01],减轻化疗后不良反应.结论:化疗联合小柴胡汤治疗乳腺癌能够提高临床有效率、T 淋巴细胞水平及免疫球蛋白含量,降低 CD8+水平,降低肿瘤标志物、肿瘤血管内皮生长因子含量,降低不良反应.
Objective:To systematically evaluate the efficacy and safety of chemotherapy combined with the classic prescription Xiaochaihu(小柴胡)Decoction in the treatment of breast cancer by meta-analysis and provide evidence-based medical evidence support.Methods:Randomized controlled studies on chemotherapy combined with Xiaochaihu Decoction for treating breast cancer that were published before January 1,2026 were searched against PubMed,Embase,The Cochrane Library,Web of Science,CNKI,Wanfang Data,VIP,and SinoMed.RevMan5.3 was used for data analysis.Results:Seventeen studies were included,with 1 292 patients.The results of meta-analysis showed that compared with chemotherapy alone,chemotherapy combined with Xiaochaihu Decoction improved the clinical response rate[RR=1.45,95%CI(1.30,1.62),P<0.01],decreased the levels of carbohydrate antigen 125(CA125)[MD=-41.26,95%CI(-53.32,-29.20),P<0.01],carbohydrate antigen 153(CA153)[MD=-15.28,95%CI(-16.81,-13.75),P<0.01],carcinoembryonic antigen(CEA)[MD=-3.73,95%CI(-4.23,-3.24),P<0.01],insulin-like growth factor-1(IGF-1)[MD=-58.09,95%CI(-59.97,-56.21),P<0.01],and soluble E-cadherin(sEC)[MD=-0.79,95%CI(-0.82,-0.77),P<0.01],increased T lymphocytes(CD3+,CD4+,and CD4+/CD8+)[MD=0.31,95%CI(0.25,0.36),P<0.01],inhibited CD8+expression[MD=-5.88,95%CI(-6.52,-5.24),P<0.01],raised the levels of immunoglobulins A,M,and G[MD=0.50,95%CI(0.47,0.53),P<0.01],lowered the levels of tumor vascular endothelial growth factors A[MD=-6.98,95%CI(-8.68,-5.28),P<0.01]and B[MD=-5.82,95%CI(-7.73,-3.90),P<0.01],and alleviated adverse reactions after chemotherapy.Conclusion:Chemotherapy combined with Xiaochaihu Decoction in the treatment of breast cancer can improve the clinical response rate,elevate the levels of T lymphocytes and immunoglobulins,inhibit CD8+expression,lower the levels of tumor markers and tumor vascular endothelial growth factors,and reduce adverse re-actions.The conclusion remains to be validated by more high-quality,multi-center,and large-sample studies.
