基于数据挖掘和网络药理学探讨雄黄用药规律及配伍防治痰瘀互结型缺血性中风机制OA

目的:基于数据挖掘技术探索含雄黄处方治"痰"的用药规律,并利用网络药理学方法分析含雄黄治"痰"核心组合防治痰瘀互结引起的缺血性中风机制,为临床配伍及用药研究提供参考.方法:基于《全国中药成药处方集》检索处方,并通过数据挖掘分析含雄黄处方治"痰"的配伍规律,分析雄黄配伍规则,以符合支持度≥0.55、置信度≥0.90且提升度≥1.00的中药为雄黄核心药物组,运用网络药理学方法分析含雄黄治"痰"核心组合防治痰瘀互结引起的缺血性中风机制,并运用CytoScape软件进行可视化处理,运用AutoDock、PyMOL等软件进行分子对接.结果:符合纳入标准的含雄黄处方共164首,涉及药物211味.高频药物(使用频次≥30)共有20味,药性以寒、温为主,药味以辛、甘、苦为主,归肝经、心经较多,筛选出5种核心药物"雄黄-麝香-朱砂-牛黄-胆南星".对核心药物组合进行网络药理学分析,前3个靶点基因分别是胰岛素(insulin,INS)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子(tumor necrosis factor,TNF).基因本体论(GO)功能富集分析和京都基因与基因组百科全书(KEGG)通路富集分析显示,核心药物组合参与收缩的心肌细胞动作电位、动作电位过程中的膜去极化、对缺氧的反应等生物过程,信号通路主要涉及脂质和动脉粥样硬化、糖尿病并发症中的晚期糖基化终末产物(advanced glycation end product,AGE)-晚期糖基化终末产物受体(advanced glycation end product receptor,RAGE)信号通路等.分子对接结果表明,INS与鹅去氧胆酸能高效结合.结论:初步分析了含雄黄方剂治"痰"的配伍规律,并且揭示了含雄黄核心药物组防治痰瘀互结引起的缺血性中风的分子机制,为后续雄黄临床配伍研究提供理论基础.

Based on data mining and network pharmacology,this study aims to explore the high-frequency medication regularity of Xionghuang(Realgar)-containing prescriptions for treating"phlegm",and analyze the mechanism of the core combinations of Xionghuang-containing prescriptions for phlegm treatment in preventing and treating ischemic stroke induced by phlegm-blood stasis intermingling,so as to provide a reference for clinical compatibility and medication research.Methods:Prescriptions were retrieved from the National Collection of Traditional Chinese Medicine Formulations.Data mining analysis was conducted to identify the combination patterns of prescriptions containing Xionghuang for treating"phlegm,"and the combination rules of Xionghuang were analyzed.The core medicinal group of Xionghuang was defined as Chinese medicinal materials that met the thresholds of support ≥ 0.55,confidence ≥ 0.90 and lift ≥ 1.00 via association rule analysis.Network pharmacology methods were used to analyze the mechanism of the core combination of Xionghuang-containing prescriptions for treating"phlegm"in preventing and treating ischemic stroke caused by the mutual obstruction of phlegm and blood stasis.CytoScape software was used for visualization,and AutoDock and PyMOL software were used for molecular docking.Results:A total of 164 prescriptions containing Xionghuang that met the data mining inclusion criteria were identified,involving 211 herbs.There were 20 high-frequency herbs(usage frequency≥30 times).The primary pharmacological properties were cold and warm,with the main flavors being pungent,sweet,and bitter.The medicines identified 5 core medicines:"Xionghuang-Shexiang(Moschus)-Zhusha(Cinnabaris)-Niuhuang(Bovis Calculus)-Dannanxing(Arisaema Cum Bile)",which primarily entered the liver and heart meridians.Network pharmacology analysis of the core drug combination identified the top three target genes as insulin,interleukin-6,and tumor necrosis factor.GO functional and KEGG pathway enrichment analyses showed that the core drug combination is involved in biological processes such as the action potential of contracting myocardial cells,membrane depolarization during the action potential process,and responses to hypoxia.Signaling pathways mainly involve lipids and atherosclerosis,as well as the AGE-RAGE signaling pathway in diabetic complications.Molecular docking results indicated that the INS can efficiently bind to Chenodeoxycholic acid.Conclusion:This study preliminarily analyzed the compatibility rules of Xionghuang-containing prescriptions for the treatment of"phlegm"syndrome,revealed the molecular mechanism of the core Xionghuang-containing herb groups in preventing and treating ischemic stroke caused by phlegm-blood stasis intermingling,and provide a theoretical basis for subsequent clinical compatibility research of Realgar.
