Beck评分口腔护理联合定时分步分段吸痰法在行气管切开机械通气患者中的应用OA
Application of Beck score-based oral care combined with timed stepwise segmented suctioning method in patients undergoing tracheotomy and mechanical ventilation
目的 分析气管切开机械通气中 Beck 评分口腔护理、定时分步分段吸痰法的联用效果.方法 选择2022 年2 月至 2024 年 9 月江苏省建湖县人民医院接受气管切开机械通气患者(n=120)为对象,根据是否接受基于 Beck 评分口腔护理联合定时分步分段吸痰法将患者分为观察组与对照组,每组均为 60 例,对照组方案为常规护理,观察组方案为基于 Beck 评分口腔护理联合定时分步分段吸痰法.对比 2 组干预后对患者口腔清洁度、炎症反应以及临床舒适度的影响.结果 干预5 d 后,观察组口腔清洁度中的I 度、Ⅱ度占比均高于对照组,Ⅲ度占比则低于对照组(P均<0.05).干预 5 d 后,观察组的白介素-6、超敏 C 反应蛋白、肿瘤坏死因子对比对照组表达水平均更低,Kolcaba舒适状况量表各项评分均更高(P 均<0.05),且观察组与对照组的指标和评分对比干预前均更优(P 均<0.05).观察组的每日吸痰次数少于对照组,干预 5 d 后痰液黏稠度低于对照组,机械通气时间较对照组更短(P 均<0.05).结论 联合使用定时分步分段吸痰法和基于 Beck 评分口腔护理,可显著改善行气管切开机械通气患者的口腔清洁效果,减少炎症反应和每日吸痰次数,降低干预 5 d 后痰液黏稠度,缩短机械通气时间,提高患者临床舒适度.
Objective To analyze the effect of Beck score-based oral care combined with timed stepwise segmented suctioning method in patients undergoing tracheostomy and mechanical ventilation.Methods A total of 120 patients undergoing tracheostomy and mechanical ventilation at Jianhu People's Hospital of Jiangsu Province from February 2022 to September 2024 were selected.The patients were divided into an observation group and a control group based on whether they accepted the Beck score-based oral care combined with timed stepwise segmented suctioning,with 60 patients in each group.The control group received routine nursing care,while the observation group received Beck score-based oral care combined with timed stepwise segmented suctioning.The effect of intervention on oral cleanliness,inflammatory response,and clinical comfort were compared between the two groups.Results After 5 days of intervention,the proportions of patients with grades I and Ⅱoral cleanliness in the observation group were higher than those in the control group,while the proportion of patients with grade Ⅲ oral cleanliness in the observation group was lower than that in the control group(P<0.05).After 5 days of intervention,the expression levels of interleukin-6,high-sensitivity C-reactive protein,and tumor necrosis factor-α in the observation group were lower than those in the control group,and the scores of all dimensions of the General Comfort Questionnaire in the observation group were higher than those in the control group(P<0.05).In both groups,these indicators showed significant improvement compared with baseline values before the intervention(P<0.05).The number of daily sputum suctionings in the observation group was less than that in the control group.After 5 days of intervention,the viscosity of the sputum in the observation group was lower than that in the control group.The duration of mechanical ventilation in the observation group was shorter than that in the control group(P<0.05).Conclusion Beck score-based oral care combined with timed stepwise segmented suctioning method can significantly improve oral cleanliness,reduce inflammatory responses and the frequency of daily suctioning,lower the viscosity of sputum after 5 days of intervention,shorten the duration of mechanical ventilation,and enhance clinical comfort in patients undergoing tracheostomy and mechanical ventilation.
