中医定向透药疗法联合加药四物汤对老年髋部骨折术后隐性失血患者的应用效果OA
The application effect of TCM directional penetration therapy combined with modified Siwu Decoction on elderly patients with hidden blood loss after hip fracture surgery
目的 探讨中医定向透药疗法联合加药四物汤食疗护理在老年髋部骨折术后隐性失血患者护理中的应用效果.方法 选取 2023 年 9 月至 2025 年 8 月期间于常州市中医医院接受治疗并出现术后隐性失血的老年髋部骨折患者150 例作为研究对象.采用随机数字表法将患者分为2 组,每组各75 例.对照组实施常规术后护理,观察组则在常规护理基础上联合应用中医定向透药疗法与加药四物汤膳食调理.比较 2 组干预前后的匹兹堡睡眠质量指数(PSQI)、疼痛视觉模拟评分(VAS)、Harris 髋关节评分(HHS)、Berg 平衡量表(BBS)、晨僵持续时间及握力水平.同时,记录并比较 2 组患者住院期间并发症发生情况,以及术后第 3 天、第 7 天的隐性失血量.结果 干预 7 d 后、随访1 个月后,观察组PSQI 及VAS 均低于对照组,HHS、BBS 均高于对照组(P 均<0.05).干预7 d 后、随访1 个月后,观察组晨僵时间均短于对照组,握力值均高于对照组(P 均<0.05).观察组并发症总发生率低于对照组,术后3 d、7 d 隐性失血量少于对照组(P 均<0.05).结论 中医定向透药疗法联合加药四物汤食疗护理可有效改善老年髋部骨折术后患者的睡眠质量,减轻疼痛,促进肢体运动与平衡功能恢复,缩短晨僵时间,增强肌力,并能降低并发症发生率,减少术后隐性失血.
Objective To evaluate the application effect of traditional Chinese medicine(TCM)directional penetration therapy combined with modified Siwu Decoction dietary care in nursing for elderly patients with hidden blood loss after hip fracture surgery.Methods A total of 150 elderly patients with hip fractures who developed postoperative hidden blood loss and received treatment at Changzhou Hospital of Traditional Chinese Medicine from September 2023 to August 2025 were selected as the research subjects.Patients were randomly divided into two groups using a random number table method,with 75 cases in each group.The control group received routine postoperative care,while the observation group received TCM directional penetration therapy and dietary regulation with modified Siwu Decoction on the basis of routine nursing.The scores of Pittsburgh Sleep Quality Index(PSQI),Visual Analogue Scale(VAS)of pain,Harris Hip Score(HHS),Berg Balance Scale(BBS),duration of morning stiffness,and grip strength levels were compared between the two groups before and after intervention.Additionally,the incidence of complications during hospitalization and the volume of hidden blood loss on postoperative days 3 and 7 were recorded and compared.Results After 7 days of intervention and 1 month of follow-up,the PSQI and VAS scores in the observation group were significantly lower than those in the control group,while HHS and BBS scores in the observation group were significantly higher than those in the control group(P<0.05).After 7 days of intervention and 1 month of follow-up,compared with the control group,the observation group had shorter morning stiffness duration and higher grip strength values(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the hidden blood loss at 3 days and 7 days after surgery in the observation group was less than that in the control group(P<0.05).Conclusion The combination of TCM directional penetration therapy and modified Siwu Decoction dietary care can effectively improve sleep quality,reduce pain,promote the recovery of limb mobility and balance function,shorten morning stiffness duration,enhance muscle strength,reduce the incidence of complications,and decrease postoperative hidden blood loss in elderly patients after hip fracture surgery.
