基于舒适化浅镇静理念的早期康复护理在ICU获得性衰弱患者中的应用OA
Application of early rehabilitation nursing based on the eCASH concept in patients with ICU-acquired weakness
目的 探讨以患者为中心的舒适化浅镇静(eCASH)理念早期康复护理在重症监护室获得性衰弱(ICU-AW)患者中的应用效果.方法 选取厦门大学附属第一医院2022 年11 月至2024 年11 月收治的ICU-AW 患者82 例进行研究,根据随机数字表法,观察组和对照组各 41 例.对照组护理方法为常规护理,观察组护理方法为常规护理结合 eCASH 理念早期康复护理.比较 2 组的营养学指标[血红蛋白(Hb)、前白蛋白(PA)以及白蛋白(ALB)],运动功能、肌力状态及生活能力.结果 护理 2 周及 4 周后,2 组医学研究理事会(MRC)评分、Barthel 指数评分、Fugl-Meyer 运动功能评分均较护理前显著提升,且观察组均高于对照组(P 均<0.05).观察组患者 Hb、PA 以及 ALB 营养学指标改善情况均优于对照组(P 均<0.05).护理 4 周后,观察组总住院天数、机械通气时间及 ICU 住院天数均短于对照组(P 均<0.05).结论 基于 eCASH 理念早期康复护理能有效增强 ICU-AW 患者的肌力状态、改善运动功能和营养水平,缩短机械通气时间和住院时间,值得临床推广应用.
Objective To explore the application effect of early rehabilitation nursing based on the patient-centered eCASH(early Comfort using Analgesia,minimal Sedatives,and maximal Humane care)concept in patients with intensive care unit-acquired weakness(ICU-AW).Methods A total of 82 patients with ICU-AW admitted at the First Affiliated Hospital of Xiamen University from November 2022 to November 2024 were enrolled in the study.They were assigned into an observation group and a control group using a random number table method,with 41 cases in each group.The control group received routine nursing,while the observation group received routine nursing combined with early rehabilitation nursing based on the eCASH concept.The nutritional indicators(hemoglobin[Hb],prealbumin[PA],and albumin[ALB]),motor function,muscle strength,and activities of daily living were compared between the two groups.Results After 2 and 4 weeks of nursing,the Medical Research Council(MRC)scores,the Barthel index,and the Fugl-Meyer Assessment scores in both groups were significantly higher than those before nursing,and the scores in the observation group were significantly higher than those in the control group(P<0.05).The improvement of nutritional indicators(Hb,PA,and ALB)in the observation group was significantly better than that in the control group(P<0.05).After 4 weeks of nursing,the observation group had significantly shorter length of total hospital stay,mechanical ventilation duration,and ICU length of stay compared with the control group(P<0.05).Conclusion The implementation of early rehabilitation nursing based on the eCASH concept can effectively enhance muscle strength,improve motor function and nutritional status,and shorten the duration of mechanical ventilation and length of hospital stay in patients with ICU-AW.It is worthy of clinical promotion and application.
