基于老年综合评估的护理在行股骨近端防旋髓内钉术股骨粗隆间骨折患者中的应用OA
Application of nursing based on comprehensive geriatric assessment in patients with intertrochanteric femoral fracture undergoing proximal femoral nail antirotation surgery
目的 探讨基于老年综合评估(CGA)的护理在行股骨近端防旋髓内钉(PFNA)术股骨粗隆间骨折(IFF)患者中的应用效果.方法 纳入 2023 年 1 月至 2024 年 12 月在南通市第三人民医院行 PFNA 术的 98 例 IFF 患者,按随机数字表法进行分组,各49 例.常规组予以常规护理,CGA 护理组在常规组的基础上加用基于CGA 的护理.观察 2 组的护理效果.结果 2 组干预 3 个月后的髋关节功能评分量表、功能性步行分级量表和一般自我效能感量表评分均高于干预前(P 均<0.05),且 CGA 护理组均较常规组高(P 均<0.05).2 组干预 1 个月和 3 个月后的视觉模拟评分法评分均降低(P 均<0.05),且 CGA 护理组低于常规组(P 均<0.05).CGA 护理组的并发症总发生率比常规组低(P<0.05).结论 基于 CGA 的护理能够改善行 PFNA 术 IFF 患者的髋关节功能、步行功能和自我效能感,并能缓解疼痛和减少并发症的发生.
Objective To explore the application effect of nursing based on comprehensive geriatric assessment(CGA)in patients with intertrochanteric femoral fracture(IFF)undergoing proximal femoral nail antirotation(PFNA)surgery.Methods A total of 98 IFF patients undergoing PFNA surgery admitted to Nantong Third People's Hospital from January 2023 to December 2024 were enrolled and divided into two groups according to the random number table method,with 49 patients in each group.The routine group received conventional nursing,while the CGA nursing group received CGA-based nursing in addition to the routine nursing.The nursing effects were compared between the two groups.Results After 3 months of intervention,the scores of the Harris Hip Score,Functional Ambulation Category,and General Self-Efficacy Scale in both groups were higher than those before intervention(P<0.05),and the scores in the CGA nursing group were higher than those in the routine group(P<0.05).The Visual Analogue Scale scores in both groups decreased after 1 and 3 months of intervention(P<0.05),and the scores in the CGA nursing group were lower than those in the routine group(P<0.05).The total incidence of complications in the CGA nursing group was lower than that in the routine group(P<0.05).Conclusion CGA-based nursing can improve hip function,walking function,and self-efficacy in patients with IFF undergoing PFNA surgery,while also alleviating pain and reducing the incidence of complications.
