膀胱过度活动症(OAB)的发病与中枢调控异常、外周神经敏化及逼尿肌功能障碍密切相关.研究表明,电针通过调节脑桥排尿中枢与高位脑区功能、抑制膀胱传入神经C纤维活性及调节自主神经平衡及神经递质(如5-HT、NO及ATP)释放,从而改善膀胱感觉与抑制异常收缩.此外,电针还具备调节逼尿肌离子通道与抗炎作用.当前研究多聚焦于单一机制,对多层次网络整合及最优刺激参数的认识仍不足.未来需结合神经影像学与多组学技术,深化电针多靶点整合机制研究,以期推动其临床精准应用.

The onset of overactive bladder(OAB)is closely associated with central regulatory dysfunction,peripheral nerve sensitization,and detrusor muscle dysfunction.Evidence suggests that electro-acupuncture(EA)can improve bladder sensory function and inhibit abnormal contractions by regulating the pontine micturition center and higher brain regions,suppressing the activity of C-fiber afferent pathways,and modulating autonomic nervous system balance as well as neurotransmitter release,including serotonin(5-HT),nitric oxide(NO),and adenosine triphosphate(ATP).In addition,EA may regulate ion channels in detrusor smooth muscle and exert anti-inflammatory effects.Current studies,however,mainly focus on single mechanisms,and understanding of multi-level network integration and optimal stimulation parameters remains limited.Future research should integrate neuroimaging and multi-omics approaches to further elucidate the multi-target regulatory mechanisms of EA and promote its precise clinical application.
