Effects of Jiaji Acupoints Catgut Embedding Combined with Treadmill Training on Spinal Cord Neural Repair and Motor Function in Rats with SCI

目的:探讨夹脊穴埋线(AE)联合跑台训练(TM)对脊髓损伤(SCI)大鼠脊髓神经修复和运动功能的影响及作用机制.方法:采用改良Allen法建立SCI大鼠模型,将造模成功大鼠随机分为SCI组、AE组、TM组和联合干预(AE+TM)组,另设置假手术(Sham)组,每组10只.对AE组、TM组和AE+TM组大鼠进行持续4周的AE和TM干预.采用BBB评分法、斜板试验和足迹分析评估各组大鼠行为学变化;多导生理记录仪测定大鼠脊髓神经传导速度;HE染色和尼氏染色观察大鼠脊髓组织病理形态改变和神经元存活情况;ELISA法检测大鼠脊髓组织中白细胞介素-1β(IL-1β)、IL-6与肿瘤坏死因子-α(TNF-α)水平;Western blot检测大鼠脊髓组织中脑源性神经营养因子(BDNF)/原肌球蛋白相关激酶B(TrkB)通路相关蛋白表达水平.结果:与Sham组比较,SCI组大鼠BBB评分、斜板角度及后爪步长和步宽均显著降低,差异具有统计学意义(P<0.05);脊髓组织中IL-1β、IL-6和TNF-α水平显著升高,差异具有统计学意义(P<0.05),神经传导速度、神经元存活数量及BDNF蛋白表达水平和p-TrkB/TrkB蛋白比值显著降低,差异具有统计学意义(P<0.05).与SCI组比较,AE组、TM组和AE+TM组大鼠BBB评分、斜板角度及后爪步长和步宽均显著升高,差异具有统计学意义(P<0.05);脊髓组织中IL-1β、IL-6和TNF-α水平显著降低,差异具有统计学意义(P<0.05),神经传导速度、神经元存活数量及BDNF蛋白表达水平和p-TrkB/TrkB蛋白比值显著升高,差异具有统计学意义(P<0.05);其中,AE+TM组大鼠脊髓神经修复及运动功能改善效果明显优于AE组和TM组,差异具有统计学意义(P<0.05).结论:AE联合TM可促进脊髓神经修复,改善SCI大鼠运动功能,其作用机制可能与激活BDNF/TrkB通路有关.

Objective:To investigate the effects and underlying mechanisms of Jiaji acupoints catgut embedding(AE)combined with treadmill training(TM)on spinal cord neural repair and motor function in rats with spinal cord injury(SCI).Methods:The SCI model was established using the modified Allen's method.The successfully modeled rats were randomly divided into the SCI group,the AE group,the TM group,and combined group(AE+TM)group,with an additional sham operation(Sham)group,with 10 rats in each group.Rats in the AE,TM,and AE+TM groups received respective interventions for 4 continuous weeks.Behavioral changes were assessed using the Basso-Beattie-Bresnahan(BBB)locomotor rating scale,inclined plane test,and footprint analysis.Spinal cord nerve conduction velocity was measured using a multichannel electrophysiological recorder.Histopathological changes and neuronal survival in spinal cord tissue were observed using HE staining and Nissl staining.Levels of IL-1β,IL-6,and TNF-α in spinal cord tissue were determined by ELISA.Western blot analysis was used to detect the protein expressions of related to the brain-derived neurotrophic factor(BDNF)/tropomyosin receptor kinase B(TrkB)signaling pathway in spinal cord tissue.Results:Compared with the Sham group,rats in the SCI group showed significantly decreased BBB scores,inclined plane angles,and hindlimb stride length and stride width(P<0.05);the levels of IL-1 β,IL-6,and TNF-α in spinal cord tissue were significantly increased(P<0.05),while nerve conduction velocity,neuronal survival,BDNF protein expression,and the p-TrkB/TrkB protein ratio were significantly decreased(P<0.05).Compared with the SCI group,rats in the AE,TM,and AE+TM groups exhibited significantly increased BBB scores,inclined plane angles,and hindlimb stride length and stride width(P<0.05);the levels of IL-1β,IL-6,and TNF-α in spinal cord tissue were significantly decreased(P<0.05),whereas nerve conduction velocity,neuronal survival,BDNF protein expression,and the p-TrkB/TrkB protein ratio were significantly increased(P<0.05).The AE+TM group showed significantly greater improvements in spinal cord neural repair and motor function than the AE group and the TM group(P<0.05).Conclusion:AE combined with TM promotes spinal cord neural repair and improves motor function in rats with SCI,and its mechanism may be related to activation of the BDNF/TrkB signaling pathway.
