34例婴幼儿听神经病临床特征及康复效果分析OA
Clinical characteristics and rehabilitation outcomes in 34 infants with auditory neuropathy
目的 分析婴幼儿听神经病(auditory neuropathy,AN)的临床特征并跟踪其康复效果,为该疾病的诊断治疗提供参考.方法 选取2015年至2021年广州医科大学附属妇女儿童医疗中心确诊为婴幼儿AN的34例患儿,其中男童20例、女童14例,双侧发病32例、单侧发病2例,确诊年龄0.25~2.67岁,平均(0.68±0.40)岁.分析病因及听力学特征,随访干预方式,并使用听觉能力分级及言语可懂度分级评估听觉言语康复效果.结果 20例(58.9%)患儿有高胆红素血症史(8例合并围产期缺氧史、7例合并早产低体质量史),其中16例出现胆红素脑病;4例有围产期缺氧史而无高胆红素血症史;2例有脑膜炎史;8例无特殊既往史,其中基因检测显示2例为OTOF基因变异、1例为TIMM8A基因变异.29例(85.3%)耳声发射通过;所有患儿均引出耳蜗微音电位;听性脑干反应均为重度以上异常.确诊2年后,11例患儿验配助听器,其中2例听觉言语能力提高,余9例无显著改善,有效率为18.2%;18例患儿行人工耳蜗植入,13例听觉言语能力提高,5例无显著改善(其中3例存在突触后病变),有效率为72.2%;5例患儿未行听力补偿治疗,其中2例单侧发病者听觉言语发育良好,3例双侧发病者无显著改善.结论 高胆红素血症是本组婴幼儿AN患者的主要致病因素,出现胆红素脑病的患儿康复效果不佳.
Objective To report clinical characteristics and rehabilitation outcomes of auditory neuropathy(AN)infants,for improving diagnosis and treatment strategies.Methods This study included 34 young children with AN(20 males,14 females;32 bilateral,2 unilateral)diagnosed at Guangzhou Women and Children's Medical Center between 2015 and 2021.The age at diagnosis ranged from 0.25 to 2.67 years(mean=0.68±0.40 years).Etiology and audiological features were analyzed,as well as rehabilitation methods.Audimetric and speech outcomes were evaluated 2 years post-diagnosis using the Category Auditory Performance and Speech Intelligibility Rate scales.Results Associated conditions included hyperbilirubinemia(n=20,58.9%)(8 with perinatal hypoxia,7 with preterm/low birth weight),bilirubin encephalopathy(n=16),perinatal hypoxia without hyperbilirubinemia(n=4)and meningitis(n=2),while there was no significant disease history in 8 cases,with 2 of them carrying OTOF and 1 carrying TIMM8A gene variants.Auditory brainstem responses were either severely abnormal or absent,despite detectable otoacoustic emissions in 29 cases(85.3%)and cochlear microphonic in all cases.Hearing aids were fitted in 11 children,with 2(18.2%)showing improved auditory-speech abilities and 9 exhibiting no significant improvement.Of the 18 children receiving cochlear implant,13(72.2%)demonstrated improvement and 5(including 3 with postsynaptic pathology)showed no notable benefits.In the 5 cases receiving no intervention,2 with unilateral AN developed normal auditory-speech abilities,while 3 with bilateral AN showed no improvement.Conclusions Hyperbilirubinemia is the primary etiological factor in this cohort of infants with AN.Patients who developed kernicterus exhibited poor rehabilitation outcomes.
