中医药调控PI3K/Akt/mTOR信号通路干预糖尿病肾病研究进展OA
Research Progress in TCM Regulation of PI3K/Akt/mTOR Signaling Pathway in the Intervention of Diabetic Kidney Disease
糖尿病肾病是糖尿病主要微血管并发症及终末期肾病重要诱因.中医药防治糖尿病肾病具有多成分、多靶点作用优势.PI3K/Akt/mTOR信号通路是调控糖尿病肾病炎症反应、自噬失衡、纤维化等病理进程的核心枢纽,与中医"本虚标实"病机存在内在契合.本文从中药有效成分、中药药对、中药复方及中成药三方面梳理中医药靶向调控PI3K/Akt/mTOR信号通路的作用机制,分析不同种类中药通过调节通路活性、改善自噬功能、抑制炎症纤维化等途径发挥肾保护作用的共性规律与特异性特征,探讨目前研究在临床转化、机制解析、质量控制等方面的局限及发展方向,为中医药治疗糖尿病肾病临床优化及创新药物研发提供参考.
Diabetic kidney disease(DKD)is a major microvascular complication of diabetes mellitus and a leading cause of end-stage renal disease.TCM offers unique advantages in the prevention and treatment of DKD through its multi-component and multi-target characteristics.The PI3K/Akt/mTOR signaling pathway serves as a central hub in regulating the pathological processes of DKD,including inflammatory responses,autophagy imbalance,and fibrosis,and aligns intrinsically with the TCM pathogenesis concept of"root deficiency and superficial excess".This review systematically summarizes the mechanisms by which TCM modulates the PI3K/Akt/mTOR signaling pathway from three aspects:active ingredients of Chinese herbs,herbal pairs,and Chinese herbal formulas and patent medicines.It analyzes the common patterns and specific characteristics through which these TCM modalities exert renal protective effects by regulating pathway activity,improving autophagy,and inhibiting inflammation and fibrosis.Furthermore,it discusses the current limitations and future directions in areas such as clinical translation,mechanistic analysis,and quality control,aiming to provide a reference for optimizing the clinical application of TCM in DKD treatment and for the development of innovative drugs.
