目的 观察芪蛭皱肺颗粒对慢性阻塞性肺疾病(COPD)大鼠IRE1α/NF-κB信号通路相关蛋白表达与炎症损伤的影响,探讨其作用机制.方法 采用香烟烟雾刺激联合气管内滴注脂多糖建立COPD大鼠模型,采用随机数字表法将60只SPF级Wistar大鼠分为空白组、模型组、补肺活血胶囊组(0.34 g/kg)和芪蛭皱肺颗粒低、中、高剂量组(0.81、1.62、3.24 g/kg),每组10只.造模结束后芪蛭皱肺颗粒低、中、高剂量组和补肺活血胶囊组分别予相应药液灌胃,空白组、模型组予生理盐水灌胃,连续28 d.HE染色观察肺组织形态,ELISA检测血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6含量,q-PCR检测葡萄糖调节蛋白78(GRP78)、肌醇需求酶1α(IRE1α)、核因子-κB(NF-κB)p65 mRNA表达,Western blot检测肺组织GRP78、IRE1α、NF-κBp65、p-NF-κBp65蛋白表达,免疫组化染色检测肺组织GRP78分布和表达.结果 与空白组比较,模型组大鼠肺泡间隔增宽、局部肺泡间隔断裂,淋巴细胞浸润明显,部分肺泡缩小甚至消失,肺泡腔代偿性扩张,肺大泡形成,支气管及血管周围纤维组织可见明显淋巴细胞浸润,支气管周围炎评分、支气管腔内渗出评分、肺泡损伤程度评分及总分显著升高(P<0.01),血清TNF-α、IL-6含量显著升高(P<0.05),肺组织GRP78、IRE1α、NF-κBp65 mRNA表达显著升高(P<0.05),GRP78、IRE1α、p-NF-κBp65蛋白表达显著升高(P<0.01),GRP78阳性表达显著升高(P<0.01);与模型组比较,芪蛭皱肺颗粒中、高剂量组及补肺活血胶囊组大鼠肺组织病理损伤减轻,支气管周围炎评分、支气管腔内渗出评分、肺泡损伤程度评分及总分降低(P<0.05,P<0.01),血清 TNF-α、IL-6 含量显著降低(P<0.05),肺组织 GRP78、IRE1α、NF-κBp65 mRNA表达显著降低(P<0.05),GRP78、IRE1α、p-NF-κBp65蛋白表达显著降低(P<0.01),GRP78阳性表达显著降低(P<0.01).结论 芪蛭皱肺颗粒可抑制IRE1α/NF-κB通路相关因子表达,阻断内质网应激介导的炎症级联反应,改善COPD模型大鼠肺损伤.

Objective To observe the effects of Qizhi Zhoufei Granules on the expression of IRE1α/NF-κB signaling pathway related proteins and inflammatory injury in rats with chronic obstructive pulmonary disease(COPD);To explore its mechanism.Methods The study used a modeling method of cigarette smoke stimulation combined with intratracheal instillation of lipopolysaccharide.Totally 60 SPF-grade Wistar rats were divided into blank group,model group,Bufei Huoxue Capsule group(0.34 g/kg)and Qizhi Zhoufei Granules low-,medium-and high-dosage groups(0.81,1.62,3.24 g/kg)according to the random number table method,with 10 rats in each group.After modeling,Qizhi Zhoufei Granules low-,medium-and high-dosage groups and Bufei Huoxue Capsule group were gavaged with corresponding drug solution respectively,and the blank group and model group were gavaged with normal saline for 28 days.The morphology of lung tissue in rats was observed by HE staining,the serum contents of tumor necrosis factor-α(TNF-α)and interleukin(IL)-6 were detected by ELISA,the mRNA expressions of glucose regulated protein 78(GRP78),inositol demand enzyme 1α(IRE1α)and nuclear factor-κB(NF-κB)p65 were detected by q-PCR,the expressions of GRP78,IRE1α,NF-κBp65 and p-NF-κBp65 proteins in lung tissue were detected by Western blot,immunohistochemical staining was used to detect the distribution and expression of GRP78 in lung tissue.Results Compared with the blank group,the alveolar septum was widened in the model group,local alveolar septum was broken,lymphocyte infiltration was obvious,some alveoli were reduced or even disappeared,the alveolar cavity was compensated for expansion,pulmonary bullae were formed,and obvious lymphocyte infiltration was observed in the bronchial and perivascular fibrous tissues,the scores of peribronchitis,intrabronchial exudation,alveolar injury and total score significantly increased(P<0.01),the contents of TNF-α and IL-6 in serum significantly increased(P<0.05),the mRNA expressions of GRP78,IRE1α and NF-κBp65 in lung tissue significantly increased(P<0.05),the protein expressions of GRP78,IRE1α and p-NF-κBp65 significantly increased(P<0.01),and the positive expression of GRP78 significantly increased(P<0.01).Compared with the model group,pathological injury of lung tissue in rats was reduced in Qizhi Zhoufei Granules medium-and high-dosage groups and Bufei Huoxue Capsule group,the scores of peribronchitis,intrabronchial exudation,alveolar injury and total score decreased(P<0.05,P<0.01),the contents of TNF-α and IL-6 in serum significantly decreased(P<0.05),the mRNA expressions of GRP78,IRE1α and NF-κBp65 in lung tissue significantly decreased(P<0.05),the protein expressions of GRP78,IRE1α and p-NF-κBp65 significantly decreased(P<0.01),and the positive expression of GRP78 significantly decreased(P<0.01).Conclusion Qizhi Zhoufei Granules can inhibit the expression of IRE1α/NF-κB pathway related factors,block the inflammatory cascade mediated by endoplasmic reticulum stress,and improve the lung injury in COPD rats.
