A case of diabetic ketoacidosis complicated with Rhizopus microsporus and severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection

患者,男性,48 岁,12 d 前无明显诱因开始出现发热,最高体温 39.2℃,伴咳嗽、咳痰.门诊新型冠状病毒抗原检测阳性,入院后,患者表现为持续高热,咳嗽、咳痰、咯血,咳嗽胸痛,查出糖尿病,肺部 CT 表现为多发病变,可见晕征、反晕征.入院第 4 天支气管肺泡灌洗液涂片发现宽大有隔菌丝,但因患者病情迅速恶化,出现呼吸困难、代谢性酸中毒,患者治疗无效死亡.后支气管肺泡灌洗液真菌培养结果为根霉属,采用基质辅助激光解吸电离飞行时间质谱和靶向测序技术鉴定为小孢根霉.临床应注意糖尿病可加速肺毛霉病的进展,应提高对肺毛霉病临床诊疗特点的认识.

A 48-year-old male patient presented with fever up to 39.2℃,accompanied by cough and expectoration,with no obvious predisposing cause 12 days prior to admission.SARS-CoV-2 antigen test was positive in outpatient setting.After admission,the patient presented with persistent high fever,cough,expectoration,hemoptysis and pleuritic chest pain.Diabetes mellitus was diagnosed.Chest CT revealed multiple pulmonary lesions with halo sign and reversed halo sign.On the 4th day after admission,broad,septate hyphae were identified in bronchoalveolar lavage fluid smears.However,the patient's condition deteriorated rapidly,complicated by dyspnea and metabolic acidosis,and the patient died due to ineffective treatment.Subsequently,fungal culture of bronchoalveolar lavage fluid yielded Rhizopus species,which was further identified as Rhizopus microsporus using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry(MALDI-TOF MS)and targeted sequencing.Clinicians should be aware that diabetes mellitus can accelerate the progression of pulmonary mucormycosis,and enhance their understanding of the clinical diagnosis and management features of pulmonary mucormycosis
