目的:优化基于故障树分析(FTA)模型的医院计算机体层成像(CT)设备维修策略,提升医院CT设备维修的准确性和有效性.方法:应用故障树模型对CT设备故障进行全面分析,通过确定顶事件、中间事件、底事件及逻辑门的应用,构建CT设备FTA模型,利用该模型对CT设备故障进行定量分析,提出优化维修策略.选取2024年1月至12月新疆维吾尔自治区人民医院白鸟湖医院和新疆维吾尔自治区人民医院两家医院临床在用的20台CT设备,按照设备管理模式不同,分别采用常规管理模式和故障树模型管理模式对CT设备进行管理,每种管理模式10台.对比两种管理模式的CT设备故障率、设备维修质量、操作人员满意度和设备维修效果.结果:采用故障树模型管理模式的10台设备30次设备故障中,CT设备自身故障率和人为操作不当故障率分别为13.33%(4/30)和16.67%(5/30),均低于常规管理模式10台设备60次故障的40.00%(24/60)和41.67%(25/60),差异均有统计学意义(x2=6.636、5.625,P<0.05),而预判故障率为90%(27/30),高于常规管理模式的58.33%(35/60),差异有统计学意义(x2=9.358,P<0.05);采用故障树模型管理模式的CT设备故障响应时间、维修时间和平均管理费用增幅均少于常规管理模式,差异均有统计学意义(t=10.938、10.909、7.074,P<0.05);CT设备操作人员对采用故障树模型管理模式的CT设备运行质量、使用操作和维修保养满意度评分均高于常规管理模式,差异均有统计学意义(t=18.470、10.629、5.347,P<0.05);采用故障树模型管理模式的CT设备平均修复率和平均可用率高于常规管理模式,差异均有统计学意义(t=10.834、13.093,P<0.05),而平均故障复发率低于常规管理模式,差异有统计学意义(t=21.452,P<0.05).结论:基于FTA模型的医院CT设备维修策略优化可显著降低设备故障率,提升设备运行质量,提高设备操作人员满意度,CT设备维修效果更佳.

Objective:To optimize maintenance strategy for computerized tomography(CT)equipment of hospital on the basis of fault tree model,and to enhance the accuracy and effectiveness of maintenance for CT equipment of hospitals.Methods:The fault tree analysis model was applied to conduct a comprehensive analysis for faults of CT equipment.By determining the top events,intermediate events,bottom events,and the application of logical gates,a fault tree model of CT equipment was constructed,which was utilized to conduct quantitative analysis for faults of CT equipment,and propose optimized maintenance strategies.Twenty CT equipment of clinical use at Bainaohu Hospital of People's Hospital of Xinjiang Uygur Autonomous Region and People's Hospital of Xinjiang Uygur Autonomous Region during January and December 2024 were selected.According to different management modes for equipment,they were managed respectively by using conventional management mode and management mode with fault tree model,with 10 equipment in each mode.The fault rate of CT equipment,maintenance quality for equipment,satisfaction of operators,and maintenance effect for equipment were compared between the two management modes.Results:In the 30 equipment faults of using management mode with fault tree model,the fault rate of CT self-equipment and the fault rate of improperly manual operation of them were respectively 13.33%(4/30)and 16.67%(5/30),all of which were lower than 40.00%(24/60)and 41.67%(25/60)of 60 faults of using conventional management mode,and the differences of them were significant(x2=6.636,5.625,P<0.05).The fault rate of predictive judgement of using management mode with fault tree model was 90%(27/30),which was higher than 58.33%(35/60)of using conventional management mode,and the difference was significant(x2=9.358,P<0.05).The increased extents of response time for fault,repair time,and average management cost of CT equipment of using management mode with fault tree model were less than those of conventional management mode,and the differences were statistically significant(t=10.938,10.909,7.074,P<0.05).The satisfaction scores of the operators of CT equipment for operation quality,usage operation,and maintenance and repair of CT equipment of using management mode with fault tree model were higher than those of using conventional management mode,and the differences were statistically significant(t=18.470,10.629,5.347,P<0.05).The average repair rate and availability rate of CT equipment of using management mode with fault tree model were higher than those of using conventional management mode,and the differences were statistically significant(t=10.834,13.093,P<0.05),while the average recurrence rate of the fault of using management mode with fault tree model was lower than that of using conventional management mode,and the difference was statistically significant(t=21.452,P<0.05).Conclusion:The optimization of maintenance strategy of CT equipment of hospital on the basis of the fault tree model can significantly reduce the fault rate of equipment,and improve operation quality of equipment,and increase satisfaction of operators,which can achieve better maintenance effect for equipment.
