增生性糖尿病视网膜病变PPV术后视力恢复的影响因素分析OA

[Objective]To analyze the factors influencing visual recovery following microincision pars plana vitrectomy(PPV)in patients with proliferative diabetic retinopathy(PDR).[Methods]A total of 212 PDR patients who underwent PPV surgery in Kaifeng Central Hospital from March 2023 to March 2025 were enrolled.Based on visual acuity 3 months postoperatively,patients were divided into a good visual recovery group(n=134)and a poor visual recovery group(n=78).Baseline data were collected.Independent risk factors influencing postoperative visual recovery and their predictive value were analyzed.[Results]The good recovery group exhibited significantly lower HbA1c levels and preoperative best corrected visual acuity(BCVA)compared with the poor recovery group.Furthermore,the proportions of patients with concurrent diabetic nephropathy,macular retinal detachment,iris neovascularization,or silicone oil tamponade were significantly lower in the good recovery group(P<0.05).Multivariate logistic regression analysis showed that HbA1c,preoperative BCVA,diabetic nephropathy,macular retinal detachment,iris neovascularization,and silicone oil filling type were independent risk factors affecting the visual recovery of PDR patients after PPV(P<0.05).Receiver operating characteristic(ROC)curve analysis confirmed that HbA1c and preoperative BCVA levels can be used to predict the visual recovery of PDR patients after PPV,and the areas under the curve were 0.884 and 0.801,respectively(P<0.05).[Conclusion]HbA1c level,preoperative BCVA,concurrent diabetic nephropathy,macular retinal detachment,iris neovascularization,and the use of silicone oil tamponade are independent risk factors influencing visual recovery after PPV in PDR patients.
