熊去氧胆酸对自身免疫性肝病患者新型冠状病毒感染的影响OA
Effect of ursodeoxycholic acid on SARS-CoV-2 infection in patients with autoimmune liver disease
目的 探究熊去氧胆酸(UDCA)对自身免疫性肝病(AILD)合并新型冠状病毒(SARS-CoV-2)感染患者的临床症状及实验室指标的影响.方法 选取2022年12月20日至2023年1月20日于中南大学湘雅医院随访的AILD患者,通过问卷调查的形式收集SARS-CoV-2感染及UDCA用药数据,并利用电子病历系统追踪感染前后肝功能指标及补体水平变化.数据采用SPSS 25.0进行统计分析.结果 UDCA组SARS-CoV-2感染率低于非UDCA组(P<0.05).对于已感染患者,UDCA对感染症状及肝损伤症状无明显改善(P>0.05).感染后,AILD患者肝酶[丙氨酸转氨酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)、岩藻糖苷酶(AFU)]及补体[补体3(C3)、补体4(C4)]水平均降低(P<0.05).与未使用UDCA组相比,UDCA治疗组胆汁淤积相关指标[总胆红素(TBil)、直接胆红素(DBil)、胆汁酸(BA)]下降(P<0.05).结论 SARS-CoV-2感染未观察到AILD患者肝损伤加重,但可能与补体系统(CS)激活及消耗有关.UDCA可降低SARS-CoV-2感染风险、改善胆汁淤积,并降低肝功能进一步损害,提示其潜在的临床保护作用.
[Objective]To investigate the effects of ursodeoxycholic acid(UDCA)on clinical symptoms and laboratory parameters of patients with autoimmune liver disease(AILD)combined with SARS-CoV-2 infection.[Methods]Patients with AILD followed up from December 20,2022 to January 20,2023 at Xiangya hospital were selected as the research subjects,SARS-CoV-2 infection and UDCA medication data in patients with AILD were collected through questionnaires,and the changes in liver function and complement levels before and after SARS-CoV-2 infection were tracked through the hospital's electronic medical record system,and SPSS 25.0 was used for statistical analysis.[Results]The UDCA group showed a significantly lower rate of SARS-CoV-2 infection than that in the non-medication group(P<0.05).For patients with SARS-CoV-2 infection,UDCA did not significantly improve infection symptoms or liver injury symptoms(P>0.05).After infection,liver enzymes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),glutamyl transpeptidase(GGT),α-L-fucosidase(AFU)]and complement levels(C3,C4)in AILD patients were significantly reduced(P<0.05).Compared with the non-UDCA treatment group,the UDCA treatment group showed a significant decrease in cholestasis-related indicators[total bilirubin(TBil),direct bilirubin(DBil),bile acid(BA)](P<0.05).[Conclusion]SARS-CoV-2 infection does not exacerbate liver injury in AILD patients but may be associated with complement system(CS)activation and comsumption.UDCA can reduce the risk of SARS-CoV-2 infection,improve cholestasis and mitigate further liver function damage,suggesting its potential clinical protective role.
