目的 本研究旨在研究聚乳酸材料经过3D打印技术处理形成的施源器在近距离后装治疗的应用,通过聚乳酸施源器与常规钛合金三管施源器的剂量学比较,得出聚乳酸施源器应用效果,为聚乳酸施源器应用提供一定的指导作用.方法 本研究为前瞻性研究,选取2024年1月至10月香港大学深圳医院收治的10例宫颈癌患者作为研究对象,患者均接受4次近距离后装治疗,每次6 Gy,其中前2次使用钛合金三管施源器(对照组),后2次采用3D打印聚乳酸施源器(试验组).通过分析比较两组高危临床靶区(HR-CTV)D90参考剂量,以及膀胱、直肠等危及器官(OAR)D2cc的剂量差异.结果 试验组采用3D打印PLA施源器的HR-CTV D90平均值为(691.2±149.2)cGy,而对照组使用钛合金三管施源器的HR-CTV D90平均值为(619.0±141.0)cGy.两组比较显示,聚乳酸施源器试验组的靶区剂量高于对照组(P<0.05).在膀胱和直肠D2cc的比较中,试验组的剂量分布更为稳定且集中,剂量均低于对照组(P<0.05).结论 3D打印聚乳酸施源器的治疗方式在宫颈癌治疗中显示出更好的靶区适形性,同时降低了直肠和膀胱的受照剂量,展现出一定的治疗优势,聚乳酸材料适用于宫颈癌治疗.

[Objective]To study the application of polylactic acid(PLA)materials processed by 3D printing technology in the treatment of brachytherapy afterloading,and to obtain the application effect of PLA applicator through the dosimetry comparison between the PLA source applicator and the conventional titanium alloy three-tube source applicator,so as to provide guidance for the application of PLA source applicator.[Methods]This study was a prospective study,and 10 patients with cervical cancer in The University of Hong Kong Shenzhen Hospital from January 2024 to October 2024 were selected,and all 10 patients were treated with 4 fractions of afterloading of 6 Gy each time,of which the first two times were treated with a titanium alloy three-tube applicator as the control group,and the last two times were treated with a 3D printed PLA source device as the test group.The D90 reference dose of high-risk clinical target volume(HR-CTV)and the D2cc dose of organs at risk(OAR)such as bladder and rectum were analyzed and compared between the two groups.[Results]The average value of HR-CTV D90 in the experimental group using the 3D printed PLA applicator was 691.2±149.2 cGy,while the average HR-CTV D90 in the control group using the titanium alloy three-tube applicator was 619.0±141.0 cGy.The comparison between the two groups showed that the target dose of the PLA applicator test group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).In the comparison of bladder and rectal D2cc,the dose distribution in the experimental group was more stable and concentrated,and the dose was lower than that of the control group(P<0.05).[Conclusion]The treatment method of 3D printed PLA applicator shows better target conformity in the treatment of cervical cancer,and at the same time reduces the radiation dose of rectum and bladder,showing certain therapeutic advantages,and PLA materials are suitable for the treatment of cervical cancer.
