鼻腔通气功能在颌面部生长发育过程中具有重要的作用.近年来,越来越多的研究证实,鼻腔结构的异常不仅会导致呼吸功能障碍,还可能通过改变气道动力学和呼吸模式,进一步影响颅面骨骼的正常发育.其中,下鼻甲体积与鼻中隔偏曲作为影响鼻气道通畅性的关键解剖因素,日益受到关注.下鼻甲体积增大会显著减少鼻腔有效通气空间,导致鼻阻力升高,诱发持续性鼻塞,迫使个体长期采用口呼吸模式;而鼻中隔偏曲则通过物理性阻塞和气流紊乱双重机制,破坏鼻腔正常通气功能,严重时还可引起对侧下鼻甲代偿性肥大,形成恶性循环.长期口呼吸会破坏口周肌功能平衡,导致舌位下降、上颌骨横向发育受限、腭盖高拱、下颌平面变陡等颅面形态异常.此外,鼻中隔偏曲还可改变软骨生长过程中产生的机械牵张力方向,干扰上颌骨及腭部的正常发育.综上所述,下鼻甲体积增大与鼻中隔偏曲不仅是导致鼻气道阻塞的常见解剖原因,更可能通过诱发口呼吸、破坏肌功能平衡及改变机械力传导等机制,对青少年颌面部的正常生长发育产生深远影响.本文将系统综述下鼻甲体积增大、鼻中隔偏曲与颌面部发育之间的相互关系以及近年来的研究进展,探讨其可能的作用机制,为追溯青少年错(牙合)畸形发生的临床病因提供参考.

Nasal airway patency plays an important role in the process of craniofacial growth and development.In recent years,a growing body of research has confirmed that structural abnormalities of the nasal cavity not only lead to respiratory dysfunction but may also further affect normal craniofacial skeletal development by altering airway dynamics and breathing patterns.Among these factors,inferior turbinate volume and nasal septal deviation,as key anatomical factors affecting nasal airway patency,have garnered increasing attention.An increase in inferior turbinate volume significantly reduces the effective space for nasal ventilation,elevates nasal resistance,induces persistent nasal obstruction,and forces individuals to adopt a long-term mouth-breathing pattern.Nasal septal deviation,through the dual mechanisms of physical obstruction and airflow turbulence,disrupts normal nasal ventilation and,in severe cases,may lead to compensatory hypertrophy of the contralateral inferior turbinate,forming a vicious cycle.Long-term mouth breathing disrupts the perioral muscular balance,leading to a lowered tongue position,restricted transverse development of the maxilla,high-arched palate,and a steeper mandibular plane,among other craniofacial morphological abnormalities.Furthermore,nasal septal deviation can alter the direction of mechanical tensile forces generated during cartilage growth,interfering with the normal development of the maxilla and palate.In summary,inferior turbinate hypertrophy and nasal septal deviation are not only common anatomical causes of nasal airway obstruction but may also exert profound effects on normal craniofacial growth and development in adolescents by inducing mouth breathing,disrupting muscular functional balance,and altering mechanical force transmission.This article systematically reviews the interrelationships and recent research progress regarding inferior turbinate hypertrophy,nasal septal deviation,and craniofacial development,explores their potential mechanisms of action,and aims to provide a reference for tracing the clinical etiology of malocclusion in adolescents.
