目的 探讨生长发育高峰期不同矢状骨面型高角患者颞下颌关节(TMJ)形态特征及性别差异,为临床正畸诊断与治疗提供参考依据.方法 选取生长发育高峰期高角患者60例,依据矢状骨面型分为骨性Ⅰ类、Ⅱ类、Ⅲ类,每组20例;采用锥形束CT(CBCT)测量关节窝、髁突及关节间隙相关形态指标,运用单因素方差分析及独立样本t检验比较各组间差异.结果 不同矢状骨面型高角患者的关节窝深度、关节结节后斜面角度、髁突前后径、髁突高度、髁突倾斜角、关节前间隙及关节后间隙存在显著差异(P<0.05);其中关节窝深度、髁突前后径、髁突高度表现为骨性Ⅲ类>骨性Ⅰ类>骨性Ⅱ类,关节结节后斜面角度表现为骨性Ⅰ类>骨性Ⅱ类>骨性Ⅲ类,髁突倾斜角、关节前间隙及关节后间隙表现为骨性Ⅱ类>骨性Ⅰ类>骨性Ⅲ类.性别比较结果显示,仅髁突内外径男性显著大于女性(P<0.05),其余TMJ形态指标性别间差异无统计学意义(P>0.05).结论 生长发育高峰期高角患者的TMJ形态受矢状骨面型影响显著,而受性别影响较小;在临床正畸诊疗中应重点关注不同矢状骨面型患者髁突形态与关节间隙的差异.

Objective To investigate the morphological characteristics of the temporomandibular joint(TMJ)in high-angle patients with different sagittal skeletal patterns during the peak growth period and to analyze the gender differences,so as to provide a theoretical basis for clinical orthodontic diagnosis and treatment.Methods Sixty high-angle patients at the peak growth period were enrolled and divided into three groups according to sagittal skeletal patterns:skeletal Class Ⅰ,Class Ⅱ,and Class Ⅲ,with 20 cases in each group.Cone-beam computed tomography(CBCT)was used to measure the morphological parameters of the glenoid fossa,condyle and joint spaces.One-way ANOVA and independent sample t-test were performed for statistical analysis.Results There were significant differences in glenoid fossa depth,posterior slope angle of articular eminence,condylar anteroposterior diameter,condylar height,condylar inclination angle,anterior joint space and posterior joint space among different sagittal skeletal patterns(P<0.05).Glenoid fossa depth,condylar anteroposterior diameter and condylar height were ranked as skeletal Class Ⅲ>Class Ⅰ>Class Ⅱ.Posterior slope angle of articular eminence was Class Ⅰ>Class Ⅱ>Class Ⅲ.Condylar inclination angle,anterior and posterior joint spaces were Class Ⅱ>Class Ⅰ>Class Ⅲ.Gender comparison showed that only the condylar mediolateral diameter was significantly larger in males than in females(P<0.05),while no significant differences were found in other TMJ parameters between genders(P>0.05).Conclusion TMJ morphology in high-angle patients during the peak growth period is significantly affected by sagittal skeletal patterns but slightly affected by gender.Attention should be paid to the differences in condylar morphology and joint spaces in patients with different sagittal skeletal patterns during orthodontic treatment.
