The prevalence of exercise-induced desaturation(EID)ranges from 5%to 62%due to varied definitions,and is significantly associated with accelerated lung function decline,a 50%increased risk of acute exacerbation,and elevated all-cause mortality.As a core respiratory rehabilitation intervention for EID in patients with chronic obstructive pulmonary disease(COPD),oxygen therapy can synergize with exercise training(e.g.,intermittent aerobic plus resistance training)to overcome EID-related limitations.Titrated oxygen therapy significantly improves exercise endurance during a single test session and enhances muscle adaptability compared to fixed-flow oxygen therapy.High-flow nasal cannula(HFNC)prolongs constant work-rate exercise time.In terms of safety,titrated oxygen therapy does not exacerbate carbon dioxide(CO2)retention in patients with hypercapnia.Future efforts should focus on identifying the populations most likely to gain benefit(e.g.,based on EID severity),optimizing closed-loop feedback technologies,and deeply integrating oxygen therapy with multidisciplinary rehabilitation programs to enhance functional independence in patients.
