轻中度慢性阻塞性肺疾病患者早期干预的人群选择策略OA
Population selection strategy for early intervention in patients with mild to moderate chronic obstructive pulmonary disease
慢性阻塞性肺疾病(简称慢阻肺病)是全球主要的公共卫生问题.轻中度慢阻肺病患者占慢阻肺病总体的90％以上,管理该群体有望显著减少整体疾病负担.早期使用噻托溴铵等药物可改善肺功能、延缓肺功能下降、减少急性加重和降低临床重要恶化风险.然而,并非所有的轻中度慢阻肺病患者都需要药物干预.由于轻中度慢阻肺病具有显著的异质性,存在不同的表型、内型和疾病演进规律,因此亟需多维度评估慢阻肺病疾病进展的风险,并对快速进展的轻中度慢阻肺病进行精准药物治疗,这既能延缓疾病进展和减少疾病负担,也可避免过度治疗.
Chronic obstructive pulmonary disease(COPD)is a major global public health issue.Patients with mild to moderate COPD account for more than 90％of the total COPD population,and managing this group is expected to significantly reduce the overall disease burden.Early use of drugs such as tiotropium can improve lung function,slow the decline in lung function,reduce acute exacerbations,and lower the risk of clinically important deterioration.However,not all patients with mild to moderate COPD require pharmacological intervention.Due to the significant heterogeneity in mild to moderate COPD,which includes different phenotypes,endotypes,and disease progression patterns,there is an urgent need for multidimensional assessment of progression risks for COPD,as well as precise pharmacological treatment for mild to moderate COPD with rapid progression.This approach could delay disease progression and reduce disease burden,while also avoiding overtreatment.
