目的 研究新型抗-CD38 迈泽妥单抗对输血前检测试验的具体干扰情况,并构建一种有效的免疫磁珠处理方法以消除该干扰,同时评估其对其他血型系统的影响及处理的时效性,为临床安全输血提供实验依据和解决方案.方法 合成装载 MOLP8 细胞蛋白的免疫磁珠.检测不同浓度迈泽妥单抗模拟标本的抗筛结果,并验证该免疫磁珠对迈泽妥单抗干扰的消除效果;评估免疫磁珠吸附迈泽妥单抗后对其他血型系统抗原抗体反应的影响;并对免疫磁珠消除干扰的时效性进行研究.结果 成功合成装载 MOLP8 细胞蛋白的免疫磁珠;免疫磁珠能有效消除不同浓度迈泽妥单抗对输血前检测的干扰,且该消除干扰同时对其他血型系统检出无影响,时效性可稳定维持 2 周.结论 合成的免疫磁珠可有效、特异地消除迈泽妥单抗对输血前血清学检测的干扰,且不影响其他血型系统的检测,有望为接受迈泽妥单抗治疗患者的安全输血提供一定的理论依据.

Objective To investigate the specific interference of the novel anti-CD38 antibody mezagitamab in pre-transfusion testing,establish an immunomagnetic bead pre-treatment method to eliminate it,and evaluate its impact on other blood group systems and clinical timeliness,thereby providing experimental evidence and a solution for safe transfusion.Methods MOLP8 cellular protein coated immunomagnetic beads were prepared.Unexpected antibody screening was per-formed on simulated samples with different mezagitamab concentrations to verify the interference elimination effect.The im-pact of immunomagnetic bead adsorption of mezagitamab on antigenantibody reactions in other blood group systems was as-sessed,and the duration of the interferenceeliminating effect was studied.Results MOLP8-coated immunomagnetic beads were successfully prepared,which effectively eliminated mezagitamab interference at different concentrations without affect-ing other blood group detection,with a stable effect for 2 weeks.Conclusion MOLP8 cellular protein-coated immunomag-netic beads efficiently and specifically eliminate mezagitamab-induced pre-transfusion testing interference without affecting other blood groups,providing a theoretical basis for safe transfusion in patients receiving mezagitamab therapy.
