非高密度脂蛋白胆固醇与高密度脂蛋白胆固醇比率与脑卒中风险之间的关联OA
Association Between Non-High-Density Lipoprotein Cholesterol to High-Density Lipoprotein Cholesterol Ratio(NHHR)and Stroke Risk
目的:探究成年人非高密度脂蛋白胆固醇与高密度脂蛋白胆固醇之比(NHHR)与脑卒中风险之间的关联.方法:选取2003年至2018年美国国家健康与营养调查中的28 827名成年参与者作为研究对象.通过多因素Logistics回归模型、限制性立方样条、亚组分析及敏感性分析探讨NHHR与脑卒中风险之间的关联.结果:共纳入28 827名参与者,其中1 091人(3.79%)报告有脑卒中史.多因素Logistics回归显示,与NHHR的最低四分位数相比,最高四分位数参与者脑卒中风险降低了42%(OR=0.58,95%CI:0.40-0.86,P=0.006).限制性立方样条结果显示 NHHR与脑卒中风险之间整体显著关联(P for overall<0.001)且存在显著的非线性关系(P for non-linearity<0.001).在拐点以下的参与者NHHR每增加1单位,脑卒中患病风险降低40%.亚组分析表明对于大学及以上教育水平、已婚、规律体力活动以及未患糖尿病的人群NHHR与脑卒中风险之间的关联更加显著(P<0.05).结论:NHHR水平与脑卒中风险呈现非线性关联,提示NHHR可能在脑血管风险分层与早期预警中发挥独特作用.
[Objective]The aim of this study was to assess the association between NHHR and stroke risk in adults.[Method]A total of 28 827 adult participants from the National Health and Nutrition Examination Survey(NHANES)from 2003 to 2018 were selected for the study.The association between NHHR and stroke risk was explored by multifactorial logistic regression modeling,restricted cubic spline,subgroup and sensitivity analyses.[Result]A total of 28 827 participants were included,of whom 1 091(3.79%)reported a history of stroke.Multifactorial logistic regression showed a 42%reduction in stroke risk in participants in the highest quartile compared with the lowest quartile of NHHR(OR=0.58,95%CI:0.40-0.86,P=0.006).Restricted cubic spline results showed an overall significant association between NHHR and stroke risk(P for overall<0.001)and a significant nonlinear relationship(P for non-linearity<0.001).Subgroup analyses showed that the association between NHHR and stroke risk was more significant(P<0.05)for those with college or higher educa-tion,married,regular physical activity,and no diabetes mellitus.For participants below the inflection point,for every one-unit increase in NHHR,stroke risk decreased by 40%.[Conclusion]NHHR levels showed a nonlinear association with stroke risk,suggesting that NHHR may play a unique role in cerebrovascular risk stratification and early warning.
