目的 探讨基于人工智能(Artificial Intelligence,AI)平台的联合病案信息质量管理系统在医疗质量管理中的作用,分析其在环境相关疾病(职业病、哮喘、慢阻肺等)监测中的应用效果.方法 采用历史对照研究设计,收集该院2022年1月至2024年6月期间的病案数据.以2023年1月AI平台正式上线为时间节点,对比分析实施前后病案信息质量、医疗质量指标及管理效率的变化.通过倾向性评分匹配(PSM)平衡组间差异,并构建多维度评价指标体系,重点评估AI平台对环境相关疾病病案质量的改善情况.结果 经倾向性评分匹配后分析显示,AI平台实施后,病案信息完整率由85.2%提升至96.8%(P<0.001),编码准确率由89.5%提升至97.3%(P<0.001),病案质量综合评分由82.4分提升至94.1分(Co-hen's d=2.04).在环境健康相关疾病方面,环境暴露史记录完整率显著提升(62.1%vs 89.3%),职业病识别准确率由71.4%提升至91.8%.此外,平均住院日缩短1.3天,病案归档时间缩短65.3%.多元回归分析证实AI平台应用是病案质量改善的主要独立预测因子.结论 基于AI平台的联合病案信息质量管理系统能有效提升病案信息质量与管理效率,特别是在环境健康相关疾病的监测和识别方面发挥重要作用,为构建现代化医疗质量管理体系提供有力支撑.

Objective To explore the application value of an integrated medical record quality manage-ment system based on an Artificial Intelligence(AI)platform in medical quality management,and to analyze its effect on monitoring environment-related diseases(e.g.,occupational diseases,asthma,and COPD).Methods A historical control study was conducted using medical record data of The First Hospital of Ninghai County from January 2022 to June 2024.Taking the official launch of the AI platform in January 2023 as the cut-off point,a comparison was made of the changes in medical record quality,medical quality indicators,and management effi-ciency before and after implementation.Propensity score matching(PSM)was used to balance intergroup differ-ences,and a multi-dimensional evaluation index system was constructed to primarily assess the role of the AI plat-form in the quality improvement of medical records for environment-related diseases.Results After PSM,the in-formation completeness rate increased from 85.2%to 96.8%(P<0.001),and coding accuracy increased from 89.5%to 97.3%(P<0.001).The comprehensive quality score improved from 82.4 to 94.1(Cohen's d=2.04).For environment-related diseases,the completeness rate of exposure history records rose from 62.1%to 89.3%,and identification accuracy of occupational diseases increased from 71.4%to 91.8%.Additionally,the average length of stay decreased by 1.3 days,and archiving time was shortened by 65.3%.Multiple regression a-nalysis results confirmed that application of AI platform was the primary independent predictor for quality improve-ment.Conclusions The AI-based system effectively improves medical record quality and management efficiency,playing a crucial role in the monitoring and identification of environment-related diseases and supporting modern medical quality management.
