DWI结合T2WI定量参数评估宫颈癌患者脉管间隙浸润和宫旁浸润的研究OA
Assessment of lymphovascular space invasion and parametrial infiltration in cervical cancer using combined DWI and T2WI quantitative parameters
目的:探讨弥散加权成像(DWI)联合T2加权成像(T2WI)定量参数在术前评估宫颈癌患者脉管间隙浸润(LVSI)及宫旁浸润(PI)中的应用价值.方法:选取2021年1月—2024年10月于本院肿瘤科接受诊疗的220例宫颈癌患者,全体患者均接受MRI检查.采用Pearson相关性分析探究DWI与T2WI定量参数间的相关性.依据术后病理结果,将患者划分为LVSI阳性组、LVSI阴性组,以及PI阳性组、PI阴性组.通过单因素分析结合Cox回归模型,明确LVSI和PI的影响因素;运用受试者工作特征(ROC)曲线评估DWI与T2WI定量参数对LVSI和PI的预测效能.结果:LVSI阳性组和PI阳性组患者的ADC值、D值及峰度值都显著低于LVSI阴性组和PI阴性组(P<0.05);而LVSI阳性组和PI阳性组患者的D*值和熵值则明显高于LVSI阴性组和PI阴性组(P<0.05).相关性分析显示,T2WI纹理特征峰度与ADC、D呈正相关,与D*呈负相关;T2WI纹理特征熵值与ADC、D呈负相关,与D*呈正相关.Cox回归分析表明,ADC、D、D*、峰度和熵值均是影响宫颈癌患者发生LVSI和PI的显著因素(P<0.05).ROC曲线分析结果显示,ADC、D、D*、峰度和熵值可用于预测宫颈癌患者的LVSI和PI状态.联合上述指标进行回归分析的预测效能显著优于DWI和T2WI定量参数单独预测(P<0.05).结论:DWI和T2WI定量参数在宫颈癌患者LVSI和PI中异常表达,可应用于预测LVSI或PI的发生,值得临床进一步研究并推广.
Objective:To evaluate the diagnostic value of diffusion-weighted imaging(DWI)combined with T2-weighted imaging(T2WI)quantitative parameters in the preoperative assessment of lymphovascular space invasion(LVSI)and parametrial infiltration(PI)in patients with cervical cancer.Methods:A total of 220 cervical cancer patients who received treatment in the department of oncology of our hospital from January 2021 to October 2024 were selected.All patients underwent MRI examinations.Pearson correlation analysis was used to investigate the correlation between DWI and T2WI quantitative parameters.Based on postoperative pathological results,patients were categorized into LVSI-positive and LVSI-negative groups,as well as PI-positive and PI-negative groups.Univariate analysis combined with Cox regression models was used to identify factors influencing LVSI and PI.Receiver operating characteristic(ROC)curves were employed to assess the predictive efficiency of quantitative parameters from DWI and T2WI for LVSI and PI.Results:The ADC values,D values,and kurtosis values of the LVSI-positive group and PI-positive patients were significantly lower than those of the LVSI-negative group and PI-negative group(P<0.05);However,the D*values and entropy values of the LVSI-positive group and PI-positive group were significantly higher than those of the LVSI-negative group and PI-negative group(P<0.05).The correlation analysis shows that the kurtosis of T2WI texture features is positively correlated with ADC and D,and negatively correlated with D*.The entropy value of T2WI texture features is negatively correlated with ADC and D,and positively correlated with D*.Cox regression analysis indicated that ADC,D,D*,kurtosis,and entropy values were all significant factors influencing the occurrence of LVSI and PI in cervical cancer patients(P<0.05).ROC curve analysis results showed that ADC,D,D*,kurtosis,and entropy values can be used to predict LVSI and PI status in cervical cancer patients.The predictive performance of regression analysis combining these indicators was significantly superior to that of DWI and T2WI quantitative parameters alone(P<0.05).Conclusion:Quantitative parameters of DWI and T2WI show abnormal expression of LVSI and PI in cervical cancer patients and can be used to predict the occurrence of LVSI or PI.Further clinical research and promotion are warranted.
