目的:分析高频彩色多普勒超声检测女性乳腺癌腋窝淋巴结的声像特征、信号分布情况及血流分级.方法:回顾性选取2020年1月—2024年2月于我院予以针对性治疗的51例乳腺癌腋窝淋巴结转移患者为转移组,选取同时间段的51例乳腺癌无腋窝淋巴结转移患者为非转移组.分析两组患者的一般资料、声像特征、信号分布情况及血流分级.结果:与非转移组相比,转移组患者淋巴结最大皮质厚度≥3 mm、长短径比值≤2、有淋巴结内钙化灶比例更高(P<0.05).转移组、非转移组门型、周围型、混合型信号分布存在差异(P<0.05).转移组、非转移组血流分级0~Ⅰ级、Ⅱ级、Ⅲ级存在差异(P<0.05).多因素Logistic分析结果显示,淋巴结最大皮质厚度(OR=2.077,95%CI=1.3 13~3.286)、长短径比值(OR=1.921,95%CI=1.099~3.359)、淋巴结内钙化灶(OR=2.000,95%CI=1.335~2.994)、信号分布(OR=2.277,95%CI=1.292~4.013)、血流分级(OR=2.036,95%CI=1.178~3.518)为女性乳腺癌腋窝淋巴结转移的危险因素(P<0.05).结论:高频彩色多普勒超声检查的声像特征、信号分布情况及血流分级为女性乳腺癌腋窝淋巴结转移的危险因素,临床可据此早期评估乳腺癌腋窝淋巴结的转移情况.

Objective:To analyse the sonographic features,signal distribution patterns,and blood flow grading in high-frequency colour Doppler ultrasound examinations of axillary lymph nodes in female breast cancer patients.Methods:This was a retrospective investigation involving 51 patients with axillary lymph node metastasis from breast cancer who were received targeted treatment at our hospital from January 2020 to February 2024(metastasis group),and 51 patients without axillary lymph node metastasis from breast cancer were enrolled during the same period(non-metastasis group).The general data,sonographic features,signal distribution,and blood flow grading of both groups were analyzed.Results:Compared with non-metastasis group,the maximum cortical thickness of lymph nodes was ≥3 mm,the ratio of length to diameter was ≤2,and the proportion of calcification in lymph nodes in metastasis group was higher(P<0.05).The signal distribution of gate type,peripheral type and mixed type was different between metastasis group and non-metastasis group(P<0.05).There were differences in blood flow grade 0~Ⅰ,Ⅱ and Ⅲ between metastasis group and non-metastasis group(P<0.05).Multivariate Logistic analysis showed that the maximum cortical thickness of lymph nodes(OR=2.077,95%CI=1.313~3.286),ratio of length to diameter(OR=1.921,95%CI=1.099~3.359),calcification in lymph nodes(OR=2.000,95%CI=1.335~2.994),signal distribution(OR=2.277,95%CI=1.292~4.013)and blood flow grade(OR=2.036,95%CI=1.178~3.518)were risk factors for axillary lymph node metastasis in women with breast cancer(P<0.05).Conclusion:The ultrasonographic characteristics,signal distribution and blood flow grade detected by high frequency color Doppler ultrasound are risk factors for axillary lymph node metastasis in women with breast cancer,which can be used to evaluate whether axillary lymph node metastasis occurs early.
