目的:探讨瘤周超声黏弹性成像参数联合C-TIRADS对甲状腺结节良恶性的鉴别诊断价值.方法:前瞻性纳入152例患者共203个甲状腺结节.所有结节于穿刺或术前接受常规超声、超声黏弹性成像及剪切波弹性成像检查,测量瘤内及瘤周(2 mm)区域的弹性参数及黏弹性参数.通过绘制受试者工作特征(ROC)曲线确定最优弹性及黏弹性参数,根据其截断值调整C-TIRADS分类,比较联合模型与单独C-TIRADS的诊断效能.结果:瘤周弹性参数最大值(S-Emax)与频散参数最大值(S-Dmax)为最优参数.C-TIRADS单独诊断的AUC为0.744,特异度为69.44%、敏感度为74.05%、准确率为72.41%;经S-Emax、S-Dmax最佳截断值调整C-TIRADS分类后,其联合模型的诊断准确率分别为81.28%和82.76%,AUC分别为0.817和0.846,均显著高于单独C-TIRADS诊断(均P<0.05),但两个联合模型之间的差异无统计学意义.结论:超声黏弹性成像技术在甲状腺结节的良恶性鉴别中具有与剪切波弹性成像技术相当的诊断效能,两者均可显著提升C-TIRADS对甲状腺结节的良恶性鉴别能力,具有重要的临床应用价值.

Objective:To explore the value of combining perinodular ultrasound viscoelastic imaging parameters with the C-TIRADS in the differential diagnosis of benign and malignant thyroid nodules.Methods:A total of 203 thyroid nodules from 152 patients were prospectively enrolled.All nodules underwent conventional ultrasound,ultrasound viscoelastic imaging,and shear wave elastography before biopsy or surgery.Elastic and viscoelastic parameters of both the intranodular and perinodular(2 mm)areas were measured.Receiver operating characteristic(ROC)curves were plotted to identify the optimal elastic and viscoelastic parameters.The C-TIRADS categories were then adjusted based on their cut-off values,and the diagnostic performance of the combined models was compared with that of C-TIRADS alone.Results:The maximum elastic parameter(S-Emax)and maximum dispersion parameter(S-Dmax)of the perinodular tissue were identified as the optimal parameters.The AUC,specificity,sensitivity,and accuracy of C-TIRADS alone were 0.744,69.44%,74.05%,and 72.41%,respectively.After adjusting the C-TIRADS categories using the optimal cut-off values of S-Emax and S-Dmax,the diagnostic accuracy of the combined models was 81.28%and 82.76%,with AUC values of 0.817 and 0.846,respectively,both significantly higher than that of the individual C-TIRADS diagnosis(all P<0.05).However,there was no statistically significant difference between the two combined models.Conclusion:Ultrasound viscoelastic imaging exhibits diagnostic efficacy comparable to shear wave elastography in differentiating benign and malignant thyroid nodules.Both techniques significantly improve the diagnostic performance of C-TIRADS,demonstrating important clinical application value.
