同体位MRI与CT模拟定位影像融合在脑胶质瘤患者精确放疗中的作用研究OA
Study on the effect of fusion of same-position MRI with CT simulation localization images fusion in precise radiotherapy of brain glioma patients
目的:研究同体位MRI与CT模拟定位影像融合在脑胶质瘤患者精确放疗中的作用.方法:前瞻性选取2021年6月—2024年9月进行治疗的脑胶质瘤患者86例为研究对象,将患者随机分为研究组及对照组,研究组患者采取同体位MRI与CT模拟定位影像融合的放射治疗,对照组患者采取常规MRI与CT模拟定位影像融合的放射治疗.比较两组患者图像的精确度、融合图像的体积以及剂量、临床靶区和计划靶区的体积以及剂量的差异.结果:研究组患者的全脑、脑干、左侧眼球、右侧眼球、左侧晶状体、右侧晶状体、左侧视神经、右侧视神经、视交叉、垂体的Dice相似指数(DSC)值显著高于对照组,右侧晶状体、左侧视神经、右侧视神经、视交叉、垂体的豪斯多夫距离(HD)值显著低于对照组.研究组与对照组的全脑、脑干、左侧眼球、右侧眼球、左侧晶状体、右侧晶状体、左侧视神经、右侧视神经、视交叉、垂体的勾画体积、勾画剂量之间的差异不存在统计学意义(P>0.05).研究组患者临床靶区体积、计划靶区体积均显著高于对照组(P<0.05),临床靶区剂量、计划靶区剂量之间的差异不存在统计学意义(P>0.05).结论:同体位MRI与CT模拟定位影像融合在脑胶质瘤患者精确放疗中有助于提升图像的精确度,但对计划靶区、临床靶区的剂量以及体积之间的差异不大.
Objective:To study the effect of the same-position MRI with CT simulation localization images fusion in pre-cise radiotherapy of brain glioma patients.Methods:In this prospective study,86 patients with brain glioma who received treatment from June 2021 to September 2024 were selected as the research objects.According to the principle of randomiza-tion,the above patients were randomly divided into the study group and the control group.The patients in the study group received radiation therapy with the same-position MRI with CT simulation localization images fusion.The control group was treated with radiation therapy combined with conventional MRI imaging and simulated CT localization images.The accuracy of the images,the volume of the fused images,and the dosimetry,clinical target,planned target,and dosimetry differences were compared between the two groups.Result:In the study group,the Dice similarity cofficient(DSC)values for the whole brain,brainstem,left eye,right eye,left lens,right lens,left optic nerve,right optic nerve,optic chiasm,and pituitary gland were significantly higher than in the control group.The Hausdorff distance(HD)values for the right lens,left optic nerve,right optic nerve,optic chiasm,and pituitary gland were significantly lower than in the control group.The sketched volume and sketched dosimetry indexes of the whole brain,brainstem,left eyeball,right eyeball,left lens,right lens,left optic nerve,right optic nerve,chiasma optic,pituitary gland were not statistically significant between the study group and the control group(P>0.05).The clinical target volume and planned target volume were significantly higher in the study group(P<0.05),but there were no statistically significant differences in the clinical target dose and planned target dose between the two groups(P>0.05).Conclu-sion:The same-position MRI with CT simulation localization images fusion helps to improve the accuracy of the image in the precision radiotherapy of brain glioma patients,but there is little difference in dosimetry and volume between the planned tar-get and the clinical target.
