惠普尔养障体呼吸系统感染的诊断挑战与治疗争议OA
Challenges and controversies in the diagnosis and treatment of Tropheryma whipplei respiratory infection
惠普尔养障体(TW)是引起惠普尔病的机会致病菌.随着分子诊断技术的发展,该病原体在呼吸道标本中的检出率日益增高,但其在呼吸系统感染中的确切致病角色仍存争议.TW 肺部感染缺乏特异症状,咳嗽最常见,影像学多表现为结节、间质改变或胸腔积液.既往主要累及消化系统的经典型惠普尔病诊断金标准为十二指肠组织学检查;但对于呼吸道孤立感染,基于支气管肺泡灌洗液(BALF)的分子检测技术已成为主流诊断手段.治疗策略已从经验性用药转向基于体外药敏的个体化方案,初始治疗多推荐易透过血脑屏障的头孢曲松或美罗培南,后续维持方案存在分歧.本综述旨在系统阐述TW 呼吸系统感染的临床诊疗研究进展.
Tropheryma whipplei(TW)is an opportunistic pathogen causing Whipple's disease.With the deve-lopment of molecular diagnostic technology,the detection rate of this pathogen from respiratory specimens has increased,but its definite pathogenic role in respiratory infection remains controversial.TW pulmonary infection lacks specific symptoms;cough is the most common symptom,and imaging examination typically reveals nodules,interstitial changes,or pleural effusion.The gold standard for the diagnosis of classic Whipple's disease which mainly affected the digestive system in the past is histological examination of the duodenum.However,for isolated respiratory infection,molecular detection technology based on bronchoalveolar lavage fluid(BALF)has become the main diagnostic method.The treatment strategy has shifted from empirical medication to personalized therapy based on in vitro drug sensitivity.Ceftriaxone or meropenem,which are able to penetrate the blood-brain barrier,are initially recommended for treatment,but there are differences in the follow-up maintenance strategies.This review aims to systematically elucidate the research progress in clinical diagnosis and treatment of TW respiratory system infection.
