目的 调查并处置某院重症监护病房(ICU)发生的疑似按蚊伊丽莎白菌医院感染暴发事件,为临床防控提供依据.方法 对2025年3月23日—4月1日该院确诊为按蚊伊丽莎白菌感染的5例患者开展流行病学调查,采集环境标本进行病原学检测,通过脉冲场凝胶电泳(PFGE)分析菌株同源性.分阶段实施干预措施,初期采取接触隔离和强化环境消毒等措施,后期对洗手池开展深度消毒,并引入荧光标记法监督清洁消毒质量.结果 5例患者均接受气管插管 或 中 心 静 脉 置 管,时空分布集中于ICU 的 B区和 C区.环境标本病原菌检出率2.38%(1/42),阳性标本来源于24床洗手池;该分离株与P1、P2菌株同属PFGE-B型,P4、P5菌株为 A型;P3分离菌为C型,提示存在多克隆传播.所有菌株对β-内酰胺类和氨基糖苷类耐药,对米诺环素和左氧氟沙星敏感.干预措施实施1周后无新增病例,环境采样结果转为阴性.结论 洗手池环境中的菌株定植可能为本次按蚊伊丽莎白菌疑似医院感染暴发的主要原因,分子分型溯源、靶向干预与过程监督相结合有助于控制此类事件.建议将ICU与水相关的设施纳入常规监测.

Objective To investigate and control a suspected outbreak of Elizabethkingia anophelis(E.anophelis)healthcare-associated infection(HAI)in the intensive care unit(ICU)of a hospital,and to provide evidence for clinical prevention and control.Methods From March 23 to April 1,2025,an epidemiological investi-gation was carried out on 5 patients who were confirmed to be infected with E.anophelis in the hospital.Environ-mental specimens were collected for pathogenic detection,and the homology of strains was analyzed by pulsed-field gel electrophoresis(PFGE).Intervention measures were implemented in different stages,initial measures were contact isolation and strengthened environmental disinfection,followed by deep cleaning of hand-washing sinks,and fluorescence-labelled method was adopted for supervising the quality of cleaning and disinfection.Results All 5 patients received endotracheal intubation or central venous catheterization,with the cases concentrated in zones B/C of the ICU.The detection rate of pathogens from environmental specimens was 2.38%(1/42),and positive speci-men came from hand-washing sink of bed 24.This isolate belonged to the type PFGE-B along with P1 and P2 strains,while P4 and P5 strains belonged to the type A.The P3 strain belonged to type C,indicating the presence of polyclonal transmission.All strains were resistant to β-lactams and aminoglycosides,but sensitive to minocycline and levoflo-xacin.After one-week intervention measures,there were no new cases and the environmental sampling results turned negative.Conclusion The colonization of bacterial strain in the hand-washing sink environment may be the main cause of the suspected E.anophelis HAI outbreak.The combination of molecular typing tracing,targeted intervention,and process supervision can help to control such event.It is recommended to include water-related facilities in the ICU into routine monitoring.
