白血病中氧化还原稳态调控机制及靶向治疗策略的研究进展OA
Advances in regulatory mechanisms and targeted therapeutic strategies of redox homeostasis in leukemia
氧化应激与抗氧化系统的动态平衡是调控白血病发生、发展及治疗响应的关键机制.活性氧(re-active oxygen species,ROS)在低浓度时可激活磷脂酰肌醇3-激酶(phosphatidylinositol 3-kinase,PI3K)/蛋白激酶B(protein kinase B,PKB/Akt)、核因子κB(nuclear factor-κB,NF-κB)等信号通路,促进白血病细胞增殖和免疫逃逸.白血病的不同亚型具有独特的氧化还原特征,其ROS水平和抗氧化系统活性存在显著差异.白血病细胞通过核因子E2相关因子2(nuclear factor E2-related factor 2,Nrf2)-Kelch样ECH相关蛋白(Kelch-like ECH-associated protein 1,Keap1)通路、谷胱甘肽代谢网络及代谢重编程,进化出强大的抗氧化防御系统,维持异常的氧化还原稳态,导致传统治疗耐药.近年来,靶向氧化还原稳态的治疗策略展现出巨大潜力,且联合治疗策略显示出协同效应.然而,其临床应用仍受限于白血病异质性、动态监测技术不足及脱靶毒性.本综述系统总结了氧化应激与抗氧化系统在白血病中的作用机制,并探讨其临床治疗的潜在应用前景.
The dynamic equilibrium between oxidative stress and antioxidant systems serves as a pivotal regu-latory mechanism in leukemogenesis,disease progression,and therapeutic responses.At low concentrations,reactive oxy-gen species(ROS)promote proliferation and immune evasion of leukemia cell through activating signaling pathways such as phosphatidylinositol 3-kinase(PI3K)/protein kinase B(PKB/Akt)and nuclear factor-κB(NF-κB).Different leukemia subtypes have unique redox characteristics with significant differences in ROS levels and antioxidant system activity.Leu-kemia cells have evolved robust antioxidant defense systems through the nuclear factor E2-related factor 2(Nrf2)-Kelch-like ECH-associated protein 1(Keap1)pathway,glutathione metabolic network,and metabolic reprogramming,thus en-abling them to maintain aberrant redox homeostasis and develop drug-resistance.In recent years,therapeutic strategies tar-geting redox homeostasis have demonstrated significant potential,and combination therapy strategies have shown synergis-tic effects.However,their clinical translation is still limited by leukemia heterogeneity,insufficient dynamic monitoring technologies,and off-target toxicity.This review has systematically summarized the regulatory mechanism of oxidative stress and antioxidant systems in leukemia and explores their potential clinical applications.
