Progress in mechanisms and therapeutic applications of CD34+cells in cardiovascular diseases
心血管疾病严重威胁人类健康.随着再生医学研究的深入,骨髓移植以及细胞疗法在心血管疾病治疗中的应用越来越广泛.其中CD34+细胞发挥着不可或缺的作用.CD34+细胞具有多向分化潜能,通过多种方式促进心脏重构和血管再生,可能在多种心血管疾病中具有临床意义.然而临床试验结果差异巨大,CD34+细胞治疗产生异常炎症反应或病理性内皮增生等不良事件,导致对CD34+细胞的治疗效果产生诸多争议.而随着单细胞测序技术和谱系示踪技术的发展,近期研究显示CD34+细胞是一类具有异质性的细胞亚群,其来源分为组织驻留的CD34+细胞和造血系统来源的CD34+细胞.这些细胞在特定微环境或特定诱导条件下朝内皮细胞、炎症细胞和成纤维细胞等多种细胞分化并发挥不同作用.因此本综述将总结CD34+细胞来源及功能,探讨其在外周血管疾病、主动脉血管疾病以及在心脏各种疾病中的分化和功能机制.
Cardiovascular diseases represent a significant threat to human health globally.With in-depth re-search in stem cells and regenerative medicine,the application of bone marrow transplantation and cell therapy in cardio-vascular diseases is becoming increasingly widespread.Among these,CD34+cells have emerged as critical therapeutic agents due to their multipotent differentiation capabilities.These cells contribute to cardiac remodeling,vascular regenera-tion,and the mitigation of ischemia and fibrosis associated with cardiovascular pathologies through diverse mechanisms,suggesting their potential clinical relevance across a range of cardiovascular disorders.Nonetheless,clinical trial out-comes have been inconsistent,with some studies reporting adverse effects such as aberrant inflammatory responses and pathological endothelial proliferation following CD34+cell therapy,thereby generating considerable debate regarding their therapeutic efficacy.The advent of single-cell RNA sequencing technology has revealed that CD34+cells constitute a het-erogeneous population,comprising tissue-resident and hematopoietic subsets.Depending on specific microenvironmental cues or induction conditions,CD34+cells can differentiate into various cell types,including endothelial cells,inflammato-ry cells,and fibroblasts,each fulfilling distinct functional roles.Therefore,in this review,we will summarize the origins and functions of CD34+cells and explore their differentiation and functional mechanisms in peripheral vascular diseases,aortic vascular diseases,and various cardiac diseases.
