Signal Mining of Prednisone-Related Adverse Events Based on the FAERS Database

目的:依据 FAERS 数据库的报告案例,对泼尼松在上市后的不良反应事件进行数据挖掘与分析,为临床合理用药提供参考.方法:收集美国 FDA 不良事件报告系统(FAERS)数据库 2004 年第 1 季度到 2024 年第 1 季度共 81 个季度的不良事件报告(adverse event reports,ADE),采用比例失衡法对 FAERS 数据库的数据进行挖掘和分析,检测上述药物的安全信号,并分析重点累及器官和高危信号.结果:获得泼尼松 ADE 共计 107 109 例次,应用3 种数据挖掘法检测出泼尼松 ADE 共同阳性信号 1312 个,累计 26 个系统/器官分类(system organ class,SOC),主要包括感染及侵染类疾病(34.38%),良性、恶性及性质不明的肿瘤(8.84%,包括囊状和息肉状),呼吸系统、胸及纵隔疾病(5.34%).信号强度前3 的 ADR 分别为组织胞浆菌性心内膜炎,结外边缘区 B 细胞淋巴瘤(MALT 型)Ⅳ和副泡状病毒感染.结论:分析显示未见泼尼松致死的阳性信号,但泼尼松不仅会产生说明书中已经标明的加重感染的不良反应,还易导致其他未注明的良性、恶性及性质不明肿瘤的发生,应引起医务工作者的关注,加强用药回访与监测.

Objective To conduct data mining on post-marketing adverse events of prednisone based on the data from the FDA Adverse Event Reporting System(FAERS),to provide evidence for its rational clinical use.Methods Adverse event reports(ADEs)covering 81 quarters from the first quarter of 2004 to the first quarter of 2024 were retrieved from the FAE-RS database.Disproportionality analysis was performed to detect safety signals.The mainly involved organ systems and high-risk signals were also analyzed.Results A total of 107 109 ADEs related to prednisone were collected,and 1,312 consist-ent positive signals were identified by three data mining methods across 26 system organ classes(SOCs).The major SOCs included infections and infestations(34.38%),benign,malignant and unspecified neoplasms(8.84%,including cysts and polyps),and respiratory,thoracic and mediastinal disorders(5.34%).The top three adverse drug reactions by signal strength were histoplasma endocarditis,stage Ⅳ extranodal marginal zone B-cell lymphoma(MALT type)and parapoxvir-us infection.Conclusion No positive signal of prednisone-related death was found in this study.Prednisone can exacerbate infections as stated in its package insert,and also increase the risk of developing unlisted benign,malignant and unspeci-fied neoplasms.These findings warrant heightened clinical vigilance,necessitating enhanced therapeutic monitoring and fol-low-up.
