总抗氧化能力与癌症患者死亡风险的关联分析-基于NHANES数据库回顾性队列研究OA
Association Between Total Antioxidant Capacity and Mortality Risk in Cancer Patients:A Retrospective Cohort Study Based on NHANES
目的:探究总抗氧化能力(TAC)与癌症患者全因死亡率之间的关系.方法:整合 NHANES 数据库中2003 至 2018 年数据,采用逻辑回归模型评估 TAC 与癌症患者结局的关联.利用 Cox 比例风险模型分析 TAC 四分位数与全因死亡风险关系.利用 Kaplan-Meier 生存曲线进行生存分析.结果:本研究共纳入 3171 名癌症患者.研究发现,癌症的发生与全因死亡率显著相关(P<0.0001).多因素 logistic 回归分析显示,在校正混杂因素后,较高的 TAC 人群(Q3 组)相较于最低组(Q1 组),全因死亡率显著降低(HR=0.76,95%CI=0.59~0.98,P=0.037).在乳腺癌患者中,Q3 组的全因死亡率显著降低(Q2 组 vs.Q1 组:HR=0.44,95%CI=0.24~0.81,P=0.008;Q3 组 vs.Q1 组:HR=0.48,95%CI=0.26~0.89,P=0.021).亚组分析显示:处于已婚或同居的癌症患者全因死亡率显著降低(交互作用 P 值=0.036).Kaplan-Meier 生存曲线表明,TAC 高水平组的总体生存率显著高于低水平组(P<0.0001).结论:在一定范围内,TAC 水平越高,癌症患者的全因死亡率越低;婚姻同居状况是调节 TAC 对全因死亡率影响的关键因素.
Objective To explore the association between total antioxidant capacity(TAC)and all-cause mortality among cancer patients.Methods Data from the National Health and Nutrition Examination Survey(NHANES)from 2003 to 2018 were integrated.Logistic regression models were used to assess the association between TAC and clinical outcomes of cancer patients.Cox proportional hazards model was used to analyze the association between TAC quartiles and the risk of all-cause mortality.Kaplan-Meier method was adopted for survival analysis.Results A total of 3171 cancer patients were en-rolled in this study.It was found that cancer was correlated with all-cause mortality(P<0.0001).Multivariate logistic re-gression analysis revealed that after adjusting for confounding factors,participants in the third TAC quartile(Q3 group)had a lower risk of all-cause mortality than those in the lowest quartile(Q1 group)(HR=0.76,95%CI:0.59~0.98,P=0.037).For breast cancer patients,higher dietary TAC intake was associated with reduced all-cause mortality(Q2 group vs.Q1 group:HR=0.44,95%CI=0.24~0.81,P=0.008;Q3 group vs.Q4 group:HR=0.48,95%CI=0.26~0.89,P=0.021).Subgroup analysis demonstrated that cancer patients who were married or cohabiting presented a lower risk of all-cause mortality,with a significant interaction effect(P=0.036).Kaplan-Meier survival curves showed that the group with high TAC levels had better overall survival than the low TAC group(P<0.0001).Conclusion Within a certain range,ele-vated TAC levels are correlated with decreased all-cause mortality in cancer patients.Marital and cohabitation status serves as a critical factor moderating the influence of TAC on all-cause mortality.
