连续神经阻滞联合脉冲射频治疗带状疱疹性神经痛及对血清miR-155-5p的影响探究OA

Objective To analyze the efficacy and safety of pulsed radiofrequency combined with continuous nerve block in the treatment of herpes zoster neuralgia(HZN),and to explore its influence on serum miR-155-5p expression.Methods A total of 106 HZN patients admitted to The First Affiliated Hospital of Bengbu Medical University from June 2021 to June 2024 were enrolled and assigned to a combined group(n=53)and a control group(n=53)using a random number table method.Both groups received conventional medication and pulsed radiofrequency,while the combined group was additional-ly treated with continuous nerve block.Changes in Visual Analog Scale(VAS)score,Pittsburgh Sleep Quality Index(PSQI),serum inflammatory factors and miR-155-5p expression before and after treatment were compared between the two groups.The incidence of adverse complications during treatment was recorded.Clinical efficacy was assessed 3 months after treatment,and the incidence of postherpetic neuralgia(PHN)was observed during the 6-month follow-up.Results The VAS and PSQI scores in the combined group were lower than those in the control group at 1 week,1 month and 3 months after treatment(P＜0.05).One week after intervention,the serum levels of IL-1β,IL-6,TNF-α and miR-155-5p in the combined group were also decreased(P＜0.05).No significant complications occurred in either group during treatment.Three months after treatment,the overall response rate was higher in the combined group(P＜0.05).During fol-low-up period,the incidence of PHN was lower in the combined group(P＜0.05).Conclusion For HZN patients,contin-uous nerve block combined with pulsed radiofrequency can further relieve pain,improve sleep quality,and reduce inflam-matory factors and miR-155-5p,thereby significantly enhancing clinical efficacy and preventing the progression of HZN to the chronic stage.
