目的 比较不同病原肺炎患儿 C 反应蛋白(CRP)、白介素-6(IL-6)与血清淀粉样蛋白 A(SAA)的表达差异,并分析三项指标与不同病原类型之间的关联.方法 回顾性纳入开封市儿童医院 2023 年 1 月至 2025 年 7 月住院肺炎患儿 160 例,按病原学分为细菌组、病毒组、支原体组与混合组.收集对比四组患儿的一般资料,并且患儿入院后 24 h完成 CRP、IL-6 和 SAA 的检测.对比不同组患儿各项指标差异,以细菌组为参考组进行多项 Logistic 回归分析,探讨CRP、IL-6 及 SAA 与不同病原类型肺炎之间的关联.结果 四组一般资料比较,差异无统计学意义(P>0.05).细菌组 CRP、IL-6 与 SAA 水平均显著高于其他三组,混合组次之,支原体组居中,病毒组最低(P<0.05).多项 Logistic回归显示,以细菌组为参考组,病毒组、支原体组及混合感染组的 CRP、IL-6 与 SAA 水平均显著降低(P<0.05);进一步分析提示,随着 CRP、IL-6 及 SAA 水平下降,患儿被归类为病毒组、支原体组或混合感染组而非细菌组的可能性增加.结论 CRP、IL-6 与 SAA 可有效反映不同病原肺炎患儿的炎症反应强度,并与病原类型分类存在密切关联,其中细菌感染患儿炎症反应最为明显.

Objective To compare the differences in the expression levels of C-reactive protein(CRP),interleukin-6(IL-6),and serum amyloid A(SAA)among children with pneumonia caused by different pathogens,and to analyze the associations between these three indicators and different pathogen types.Methods A total of 160 hospitalized children with pneumonia admitted to Kaifeng Children's Hospital from January 2023 to July 2025 were retrospectively enrolled.According to etiology,they were divided into a bacterial group,a viral group,a Mycoplasma group,and a mixed infection group.The general clinical data of the four groups were collected and compared.CRP,IL-6,and SAA were measured within 24 h after admission.Differences in these indicators among the groups were compared.Using the bacterial group as the reference category,multinomial Logistic regression analysis was performed to investigate the associations of CRP,IL-6,and SAA with pneumonia caused by different pathogen types.Results There were no statistically significant differences in general data among the four groups(P>0.05).The levels of CRP,IL-6,and SAA in the bacterial group were significantly higher than those in the other three groups,followed by the mixed infection group,the Mycoplasma group,and the viral group,which had the lowest levels(P<0.05).Multinomial Logistic regression analysis showed that,with the bacterial group as the reference category,the levels of CRP,IL-6,and SAA were significantly lower in the viral group,Mycoplasma group,and mixed infection group(P<0.05).Further analysis suggested that decreases in CRP,IL-6,and SAA levels were associated with an increased likelihood of being classified into the viral group,Mycoplasma group,or mixed infection group rather than the bacterial group.Conclusion CRP,IL-6,and SAA can effectively reflect the intensity of inflammatory responses in children with pneumonia caused by different pathogens and are closely associated with pathogen classification,with the most pronounced inflammatory response observed in children with bacterial infection.
